COVID-19 감염 이후 발생한 양안 시신경병증 1예

국문 초록

목적: COVID-19 감염 이후 발생한 양안 시신경병증을 경험하였기에 이를 보고하고자 한다. 증례요약: 34세 여자 환자가 내원 16일 전 COVID-19에 감염된 후 발생한 양안 안구통증, 비문증과 시력저하로 내원하였다. 초진 시최대교정시력은 우안 0.8, 좌안 0.7이었고 좌안에 상대구심동공운동장애가 관찰되었으며 시야검사 상 양안의 맹점이 확대된 소견을보였다. 안저검사에서 양안 유두부종, 양안 시신경 주변의 망막출혈과 좌안에 경도의 유리체출혈이 관찰되었다. 형광안저혈관조영술에서는 양안 시신경유두의 전반적인 형광누출과 망막출혈에 의한 저형광이 확인되었다. 뇌 자기공명영상검사 상 좌안 시신경 경계가불분명하게 조영증강되었다. 양안 시신경병증이라는 진단 하에 고용량 스테로이드를 하루 1 g씩 3일간 정맥 투여하고 경구 스테로이드를2주간 복용하며 줄였다. 치료 1달 후 양안 시신경유두부종과 망막출혈은 모두 호전되었으며 최대교정시력 또한 1.0으로 호전되었다. 결론: COVID-19 감염 후 발생한 양안 시신경병증에서 스테로이드 치료 후 호전되었다.

영문 초록

Purpose: To report a case of bilateral optic neuropathy after COVID-19 infection Case summary: Thirty-four-year-old woman presented with bilateral ocular pain, visual disturbance, and decreased visual acuity after contracting COVID-19 infection 16 days ago. At the initial visit, best-corrected visual acuity was 0.8 in the right eye, and 0.7 in the left eye. Left eye had relative afferent papillary defect and visual field tests revealed enlarged blind spot in both eyes. Fundus exam showed bilateral optic disc swelling and peripapillary retinal hemorrhage along with vitreous hemorrhage in the left eye. Fluorescein angiography revealed bilateral leakage of dye from the optic disc with area of hypofluorescence corresponding to retinal hemorrhage around the optic disc in both eyes. Brain magnetic resonance imaging (MRI) showed ill-defined left optic nerve enhancement. A diagnosis of bilateral optic neuropathy was made, and the patient was treated with high dose IV steroid 1 g per day for 3 days and oral steroid tapering for 2 weeks. Disc swelling and retinal hemorrhage were improved 1 month after treatment along with enhanced visual acuity to 1.0. Conclusions: Bilateral optic neuropathy following COVID-19 infection showed improvement after steroid treatment.
