사회적 배제 변화가 노인의 건강변화와 건강불평등에 미치는 영향

국문 초록

본 연구의 목적은 노인의 사회적 배제와 주관적 건강상태, 우울 간의 인과관계를 검증하고, 사회적 배제가 주관적 건강상태및 우울에 미치는 종단적 영향에서 빈곤 노인과 비빈곤 노인 간 차이를 살펴보는데 있다. 이를 통해 노인들이 건강한 노년기를 보낼수 있도록 하는 지원방안을 모색하고, 노년기 건강격차를 완화할 수 있는 방안을 제시하고자 하였다. 본 연구를 위해 한국복지패널7차(2012년)에서 14차(2019년)까지의 자료를 사용하였으며, 65세 이상 노인 3,033명의 자료를 최종 분석에 활용하였다. 자료의 분석은크게 잠재성장모형과 다집단분석의 두 종단적 방법을 사용하였다. 주요 연구 결과는 다음과 같다. 첫째, 시간의 흐름에 따라 경제적배제, 주거 배제, 사회활동 배제는 점차 감소하고, 사회적 관계 배제는 일정한 수준을 유지하는 것으로 나타났다. 그리고 주관적 건강상태와 우울은 점차 악화하는 것으로 확인되었다. 둘째, 경제, 주거, 사회활동, 사회적 관계 배제가 큰 폭으로 감소할수록 주관적 건강의감소폭이 줄어들고, 우울의 증가폭 역시 줄어드는 것으로 나타났다. 셋째, 노년 초기의 사회활동 배제 수준이 노년기 삶에 지속해서영향을 미쳐 노인집단 내 주관적 건강 및 우울 수준의 격차를 유발하는 것으로 나타났다. 넷째, 빈곤 노인과 비빈곤 노인집단 간 주관적건강의 격차는 시간의 흐름에 따라 줄어드는 것으로 나타난 반면, 우울 수준의 격차는 일정한 간격을 유지하며 고착화되는 경향을보였다. 다섯째, 사회적 관계 배제 변화가 주관적 건강상태 및 우울 변화에 미치는 영향은 빈곤 노인과 비빈곤 노인 간 다른 양상을보였고, 특히 이러한 관계는 빈곤 노인집단에서 더 효과가 큰 것으로 나타났다. 이상의 연구결과를 바탕으로 노인의 사회적 배제 완화를위한 사회복지적 차원에서의 방안을 제언하였다.

영문 초록

The purpose of this study is to identify the causal relationship among social exclusion with subjective health status and depression in the elderly, as well as the differential effects of social exclusion on changes of subjective health status and depression between poor and non-poor elderlies. Through this, this study is to find the ways to help the elderly spend a healthy life in their old age and to suggest the ways to alleviate elderlies’ health disparities. Therefore, this study analyzed 3,033 elderlies aged 65 years or older who participated in the Korean Welfare Panel Study from 2012(7th) to 2019(14th). The analyses were mainly based on two longitudinal methods, multi-variate and multi-group latent growth model. The main results of this study were as follows. First, regarding the results of the longitudinal change of elderlies’ social exclusion, exclusion of economy, housing and social activities gradually decreased, while exclusion of social relationships remained at a certain level. Also, changes of subjective health status and depression in the elderlies progressively deteriorated. Second, as the exclusion of economy, housing, social activity and social relationship decreases significantly, increase in subjective health status and reduction in depression were shown. Third, exclusive level of social activities during the early years of old age continuously affected the entire life of an elderly which caused disparities in both subjective health status and depression level. Fourth, subjective health status disparities between poor and non-poor elderly groups steadily narrowed down as time passed. However, in the case of depression, disparity between the two groups tend to remain at constant intervals. Fifth, the impact of exclusion of social relationship changes on subjective health status and depression was different between the poor and the non-poor, especially these associations were found to be more effective in the poor elderly group. Based on these findings, this study suggested social welfare dimensional methods to alleviate social exclusion of the elderly.
