전정-고유수용감각훈련이 발달장애아의 상동행동에 미치는 효과

국문 초록

본 연구는 전정-고유수용감각 훈련이 발달장애아의 상동행동에 미치는 효과를 알아보는 연구이며, 연구 대상은 장애 전담 어린이집에 소속된 발달장애아로서 연구 목적에 맞는 상동행동을 가지고 있는 대상아 3명을 선정하였다. 전정-고유수용감각 훈련 프로그램은 Ayres(1985)와 坂本(1990)이 제시한 감각통합훈련 프로그램과 본 연구자의 오랜 임상 경험을 토대로 발달장애아의 상동행동 개선에 적합하다고 생각한 전정-고유수용감각 훈련 프로그램의 3가지 유형을 사용하였다. 전정-고유수용감각기능 개선은 중추신경계의 감각투입과 처리를 원활하게하고, 감각 회로 또한 일반적인 반응을 일으키게 되면, 자연적으로 상동행동 개선을 가져올 것이라는 전제하에 실시하였다. 실험 결과 전정감각이상으로 나타나는 상동행동인 ‘천정 보면서 제자리 뛰기’는 기초선 단계에서 22~19회이던 것이 0회로 완전 소멸되었으며, ‘높은 곳 올라가기’는 기초선 단계에서 30~27회이던 것이 5회로 감소하였으며, 그 외의 표적 상동행동 전반에서 효과가 있었다. 실험결과에 대한 결론으로는 전정-고유수용감각 훈련은 발달장애아의 전정감각 이상으로 나타나는 상동행동과 고유수용감각 이상으로 나타나는 상동행동 발생빈도를 감소시켰을 뿐만 아니라 전정-고유수용감각 이상과 무상관으로 나타나는 상동행동에도 효과가 있었다는 것과, 표적상동행동이 한정되어 있으므로 인하여, 그 효과를 일반화 하는데 한계가 있다는 제언을 하였다.

영문 초록

This study aims to examine the effect of the vestibular-proprioceptive sensory training on the stereotypic behavior of children with developmental disorders, and the 3 children with developmental disorders attending a special pre school exclusive for disabled children and showing the stereotypic behaviors fit for the purpose of the study were selected as the objects of the research. For the vestibular-proprioceptive sensory training program, 3 types of programs were used: the sensory integration programs presented by Ayres (1985) and Sakamoto(坂本) (1990) and the vestibular-proprioceptive sensory training program that is thought appropriate for the improvement of the stereotypic behaviors of children with developmental disorders based on the long-time clinic experience of the authors of this thesis. The preposition of the study was that the improvement in the vestibular-proprioceptive sensory function will smooth the sense input and treatment of the central nervous system and bring the normal reaction of the sensory circuit and it will naturally improve the stereotypic behaviors. The results of the experiment showed that it was effective in the general target stereotypic behaviors that appear due to the vestibular disorders: the ‘standing jump looking at the ceiling’ which used to be 22~19 times at baseline completely disappeared to 0 (zero), and the ‘climbing to a high place’ that used to be 30~27 times at baseline decreased to 5 times. The conclusion of the results of the experiment is that the vestibular-proprioceptive training not only reduced the frequency of occurrence of the stereotypic behaviors that appear due to the vestibular sensory and proprioceptive sensory problems of children with developmental disorders but also was effective for the stereotypic behaviors that are unrelated with the vestibular-proprioceptive sensory problems. It is suggested that there is a limitation to the generalization of the effects due to the limited target stereotypic behaviors.
