지방의료원 정책의 정치적 동학

국문 초록

최근 지방정부 차원의 감염병 대응 및 공공의료 서비스 제공과 관련하여 지방의료원의 중요성이 강조되고 있다. 하지만 지방의료원 설립은 수익성과 공공성 간의 충돌 가능성, 지방의료원의 필요성을 둘러싼 이념적 대립 등 정치적 복잡성으로 인해 그 추진이 결코 용이하지 않으며, 실제로 적지 않은 지방자치단체들이 지방의료원 신설에 큰 어려움을 겪고 있다. 이에 본 연구는 지방의료원 설립 정책을 둘러싼 정치적 동학을 파악하고, 지방의료원 설립의 성공요인을 발견 하기 위해 성남시의료원 설립 사례를 선정하였으며, 분석을 위해 Kingdon(1995)의 다중흐름 모형과 Moore(1995)의 정치적 관리(political management)를 결합한 이론적 분석틀을 활용 하였다. 분석 결과, 주민발의를 통해 정책문제로 등장한 성남시의료원 설립 정책은 수익성과 공공성의 대립과 같은 정치의 흐름, 다양한 조례를 통한 정책대안의 흐름이 존재하였다. 이후, 정책기업가의 등장으로 정책의 창이 열리고 정책기업가이자 공공관리자는 SNS 활용을 통한 기업 가적 옹호 전략, 효과적인 정책개발관리, 공개 토론회를 통한 협상과 공적 숙의의 장 마련 등 다양한 정치적 관리 전략을 활용함으로써 지방의료원 설립을 실현하였다. 성남시의료원 설립 정책의 성공적 추진 사례를 분석한 본 연구는 정치적 갈등이 첨예한 정책의 성공을 위해 상황에 맞는 다양한 정치적 관리 전략의 활용이 요구된다는 실천적･관리적 함의를 제공한다.

영문 초록

Recently, the necessity of regional public hospital policy has been emphasized in relation to the response to infectious diseases and provision of public medical services at the local government level. However, despite this necessity, the establishment of regional public hospital is a difficult policy to realize due to the complex political environment, such as the value conflict between profitability and publicity, and ideological conflict. Therefore, this study selected the case of establishment of Seongnam Citizens Medical Center in order to grasp the political dynamics surrounding the policy of regional public hospital and discover the success factors of the policy. This study also utilized integrated approach that composed of the multiple stream framework of Kingdon(1995) and the political management perspective of Moore(1995). As a result of the analysis, the establishment of Seongnam Citizens Medical Center had a flow of politics such as a conflict between profitability and publicity, and a flow of policy alternatives through various ordinances. After that, the emergence of policy entrepreneurs opens the window of policy, and he used various political management strategies, such as entrepreneurial advocacy strategies through the use of SNS, effective policy development management, negotiations through public debates, and public deliberation. This study presents practical and managerial implications that the use of various political management strategies appropriate to the situation is required for the success of policy with political conflicts.
