재활의료기관 시범사업기관에서의 의료사회복지업무 현황: 사례연구

국문 초록

본 연구는 재활의료기관 시범사업 기관의 사회복지실 업무현황을 파악하고 이후의 본 사업수행을 위한 보완점을 모색하는 것을 목적으로 하였다. 이를 위해 2019년 12월 한 달 동안 세 곳의 기관에서 수행된 사회복지실의 업무내용을 제공받아 분석하였다. 연구에서는 사회복지서비스가 제공된 환자의 일반적 특성인 성별, 연령, 진단유형, 발병일, 사회보험, 개인보험, 장애판정 및 요양등급에 대해 확인하였다. 사회복지업무는 개인력 조사(MM141), 사회사업상담(MM142)과 추가상담, 가정방문(MM143), 그리고 시범사업 항목은 통합계획관리, 입원기간, 퇴원 시 수정바델지수, 퇴원 장소, 집으로 퇴원 시연계기관, 돌봄 제공자의 현황을 확인하였다. 보완점으로, 사회복지실은 환자의 재입원 최소화와 독립적 생활유지를 위해 ‘돌봄, 환경개선, 재활서비스’ 외에도 재활동기 강화 등 심리사회적 지원방안 모색이 필요하다. 제도적으로는 실무자가 ‘연계를 위해 과정에서 수행하는 활동’과 더 많은 어려움에 더 많은 서비스를 제공하는 ‘추가상담’이 업무수행으로 인정 될 수 있는 수가체계의 보완이 필요하다. 그리고 재활서비스의 공급과 범위의 확대, 제공방법의 다양화를 통해 지역 별 공급편차를 완화하고 COVID 19와 같은 상황에서도 안정적으로 제공되도록 보완이 필요함을 제언하였다. 현장의 사례와 현황을 파악하고 보완점을 모색하는 이러한 시도는 이후 사업에 참여하는 다른 재활병원들이 참고할 수 있는 중요한 자료가 될 수 있을 것이다.

영문 초록

The purpose of this study is to figure the status out of social work office of institutions participating in Pilot Project of Rehabilitation Medical Institutions and to find supplements for the subsequent performance of this project. For this, three institutions were provided with the details of the social work office conducted during the month of December 2019. In the study, it was confirmed that the general characteristics of patients. Social welfare work examined the status of personal history surveys, social work counseling, additional consultations, home visitation, and pilot projects: integrated plan management, length of hospitalization, the revision Barthel Index at discharge, discharge place, association agency when discharging home, and care provider. As a complement, the social welfare office needs to seek psychological and social support, such as strengthening the re-active system, in addition to “care, environmental improvement, and rehabilitation services” to minimize patient readmission and keep living independently. Institutionally, it is necessary to supplement the medical fee in which “activities performed by practitioners in the process for cooperation” and “additional consultations” that provide more services for various difficulties can be recognized as work. And the expansion of the supply and scope of rehabilitation services, through the diversification of the delivery method to mitigate supply differences by region and it is necessary to supplement so that it is reliably provided in situations such as COVID 19.
