접형동 점액낭종으로 인한 시신경병증의 치험 1예

국문 초록

목적: 접형동 점액낭종으로 인한 시신경병증에서 비내시경적 배액술을 시행한 후 합병증 없이 신속하고 완전하게 시력 회복을 획득한 1예를 보고하고자 한다. 대상과 방법: 57세 남자 환자가 1년 전부터 발생한 좌안의 시력저하를 주소로 내원하였다. 이전 병력은 없었으며 수술 과거력도 없었다. 좌안 최대교정시력은 0.2이었고 동공은 경도로 산대되어 있었으나 상대구심성동공운동장애는 없었으며 안저는 정상이었다. 결과: 안통, 안구돌출, 외안근마비 소견은 없었고 골드만 시야검사상 하비측에서 국소적인 주변시야위축을 보였으며 시유발전위검사상 좌안에서 진폭이 감소된 소견이 나타났다. 뇌자기공명영상촬영상좌측 접형동 및 두개기저의 일부를 침범한 26.9×27.1×33.1 mm 크기의 경계가 매끈한 낭종이 안와첨 및 시신경에 인접하고 두개기저의 일부를 침범하여 뇌실질을 압박하는 소견을 발견하였다. 비내시경으로 낭종을 확인하여 국소마취하에 조대술을 통해 배액하였다. 술 후 1병일째 환자의 좌안 나안시력은 1.0으로 회복되었고 안구 후 혈종이나 외안근 손상 등의 합병증은 발생하지 않았다. 결론: 만성적인 시력저하를 동반한 접형동 점액낭종이라도 고식적인 배액술을 통해서 좋은 시력예후를 기대할 수 있었다.

영문 초록

Purpose: To report of a case of optic neuropathy due to sphenoidal mucocele that was treated with drainage under nasal endoscopy successfully. Methods: A 57-year-old male visited our ophthalmologic clinic complaining of decreased visual acuity with no ocular pain in his left eye for a year. The best corrected visual acuity was initially 1.0 in the right eye and 0.2 in the left eye. Light reflex was intact and no relative afferent papillary defect was found, but his pupil was slightly dilated. Fundus examination revealed no remarkable findings. No exophthalmos, ptosis, extraocular muscle movement limitation were found. Results: A notch-like focal contraction was found in Goldmann perimeter, and the decreased amplitude in visual evoked potential. Orbital MRI showed a 26.9×27.1×33.1 mm sized well-demarcated cystic mass involving left sphenoid sinus and skull base. The orbital apex and optic nerve was contiguous with the cystic mass. Adjacent skull base was remodeled and brain parenchyma was compressed. Incisional drainage of the cystic mass was performed by nasal endoscopy. Next day, his visual acuity became 1.0 with no significant complications such as retrobulbar hemorrhage, ptosis, or extraocular muscle movement limitation. Conclusions: Conventional incisional drainage through nasal endoscopy can be effective treatment of the mucocle of sphenoid sinus causing chronic visual disturbance.
