조기 진단과 치료에 의한 Klebsiella Pneumoniae 내인성 안내염 1예

국문 초록

목적: Klebsiella pneumoniae에 의한 내인성 안내염에서 조기 진단과 치료로 좋은 예후를 보인 1예를 문헌과 함께 보고하고자 한다. 증례요약: Klebsiella pneumoniae에 의한 간농양으로 본원 내과에서 입원 2일째인 52세 여자 환자에서 좌안 통증과 시력감소가 발생한 당일 안과적 검사와 치료가 시작되었다. 좌안 전방에서 검체를 채취하여 시행한 배양검사에서 Klebsiella pneumoniae가 동정되었다. 안과증상 발생 2일전부터 내과에서 전신적 항생제 주사로 치료 중이었으며, 안과증상이 발생한 당일 유리체 내 항생제 주사 및 항생제 점안액으로 치료하였다. 10일 후, 21일 후 유리체 내 항생제 주사를 2회 추가 시행한 결과 염증이 치유되었으며 교정시력 0.8로 시력도 보존되었다. 결론: 간농양 환자에서 발생한 Klebsiella Pneumoniae 내인성 안내염 환자의 경우 안과의사와 내과의사의 협력 하에 조기 진단과 치료가 시력 예후에 중요한 영향을 미칠 것으로 생각된다.

영문 초록

Purpose: To Report and review a case Prognosis of Klebsiella Pneumoniae endogenous endophthalmitis by early diagnosis and treatment. Case summary: A-52-year-old female hospitalized in internal medicine department for 2 days with liver abscess by Klebsiella pneumoniae was referred to us in consultation on the day of patient first complained her ocular pain and visual loss in her left eye. Ophthalmologic evaluation and antibiotic treatment were performed. Culture revealed Klebsiella pneumoniae from anterior chamber of the left eye. Systemic antibiotics, intravitreous antibiotics injection and antibiotic eye drops were performed. Additional intravitreous antibiotic injection were performed twice on day 10 and day 21, and the inflammation was cured and corrected visual acuity conserved 0.8. Conclusions: Early diagnosis and treatment by cooperation of physicians (internist) and ophthalmologists of Klebsiella pneumoniae endogenous endophthalmitis in liver abcess patients have an important effect on the prognosis of visual acuity.
