항정신병 약물 장기 복용 후 양안 각막 기질 색소침착 및 백내장이 발생한 2예

국문 초록

목적: Chlorpromazine 장기 복용 후 안구독성이 발생한 2예를 문헌고찰과 함께 보고하는 바이다. 대상과 방법: 정신분열증 진단 후 chlorpromazine을 장기 복용 중인 환자에서 시력 감소를 호소한 두 환자가 내원하였다. 결과: 양안 각막의 내측 기질, 데스메막, 내피에 걸쳐 과립상 색소가 침착되어 있었으며, 시축을 침범한 방사상 전낭하 피질 성상 백내장을 보였고, 각막내피세포수가 감소한 소견을 보인 2예를 보였다. 결론: Chlorpromazine 총복용량에 비례한 비가역적인 색소 침착이 발생하므로, 정기적 안과 검진이 필요하며, 정신과적 상태에 따라 가능한 약제의 변경이 고려되어야 한다.

영문 초록

Purpose: To report two cases of ocular changes after long term use of chlorpromazine. Methods: Two cases were presented with decreased visual acuity after long term use of chlorpromazine due to schizophrenia. Results: They showed corneal granular pigment deposition in post. stroma, Descemet membrane and endothelium, stellate ant. subcapsular cataract involving visual axis, and also decreased corneal endothelial cell count, which were persistent, although the chlorpromazine was changed into olanzapine. Conclusions: In cases of patients taking chlorpromazine, the regular ocular examination should be considered, because ocular pigment deposition occurs according to the accumulative dosage of the medicine.
