라식 후 발생한 상피내생에서 각막편 봉합 치료가 관해 후 굴절에 미치는 영향

국문 초록

목적: 라식 후 발생한 상피내생에서 각막편 봉합 치료가 관해 후 굴절에 미치는 영향을 3예에서 보고하고자 한다. 대상과 방법: 라식 수술 받은 44세 여자 환자가 우안 심부 각막염 치료 중 발생한 우안의 상피내생으로 상피내생 제거 후 각막편을 이중 연속봉합하였고, 라식 수술을 받은 28세 남자 환자가 우안 수상 후 상피 내생이 발생하여, 내생된 상피를 제거 후, 각막편을 이중 연속봉합 및 재발에 따른 추가 단속봉합을 하였다. 라식 수술을 받은 44세 여자가 우안 수상 후 중심부 각막편 손상 및 상피내생이 발생하여, 접혀진 각막편을 들고 상피내생 제거 후 손상 부분을 단속봉합하였다. 세 증례의 술 전 구면대응치와 각막지형도상 난시 값은 각각 평균 －2.92D, 3.97D였으며, 관해 후에는 －1.38D, 0.6D로 감소하였다. 결과: 본 증례들에서 라식 후 발생한 상피내생의 치료로 이중 연속 또는 단속 각막 봉합술은 상피내생으로 발생한 근시 및 난시를 효과적으로 줄이며, 봉합에 의한 추가 근시 및 난시는 발생시키지 않는 것으로 사료되었다.

영문 초록

Purpose: To report the effect of flap suturing on the refraction in the treatment of 3 cases of epithelial Ingrowth after Laser in Situ Keratomileusis (LASIK). Methods: A 44-year-old female who received LASIK a year ago, with epithelial ingrowth on her right eye was referred to our clinic. The patient had undergone removal of epithelial ingrowth and double continuous flap suturing. A 28-year-old who received LASIK 4 years ago, with epithelial ingrowth on his right eye after ocular trauma, had undergone removal of epithelial ingrowth, double continuous flap suturing, and recurred epithelial ingrowth removal and additional interrupted flap suturing. A 44-year-old female who received LASIK 10 years ago, with central flap damage and epithelial ingrowth on her right eye after ocular trauma, had undergone repositioning of the flap, removal of epithelial ingrowth and interrupted flap suturing. The preoperative average of spherical equivalent and astigmatism on the topography of these cases are －2.92D, 3.97D, which was reduced to －1.38D, 0.6D after remission. Results: The flap suturing in the treatment of epithelial Ingrowth after LASIK can effectively reduce refractive error such as myopic drift and astigmatism, do not induce additional astigmatism due to corneal suture.
