거동불편 대상자를 위한 방문진료의원 모형

국문 초록

본 논문은 방문진료의원이 거동불편 대상자를 위한 효과적인 돌봄모형이 될 수 있을지를 탐색하기 위해 수행된 사례연구이다. 인구 고령화 추세에 따라 만성질환과 장애를 복합적으로 갖고 있는 거동불편 대상자의 수가 급속히 증가할 것으로 전망된다. 이들의건강문제를 효과적으로 해결하기 위해서는 의사가 직접 환자의 집으로 찾아가는 형태의 돌봄모형을 고려하지 않을 수 없다. 건강의집의원은 2019년 3월 두 명의 의사와 한 명의 간호사에 의해 개설되었다. 이들은 방문진료만을 실시하고 외래진료를 수행하지 않고 있으며, 지금까지 총 292명의 거동불편 대상자에게 2,070건의 방문의료서비스를 제공하였다. 이 과정에서 방문진료의원은 높은 환자만족도, 가정 내 임종지원, 요양시설 입소예방, 다양한 방문진료 서비스 유형 개발, 지역사회 돌봄네트워크 구축 등의 운영성과를 보여주었다. 결과적으로 건강의집의원은 전통적인 의원과 전혀 다른 운영구조를 갖고 있음에도 불구하고 성공적인 돌봄모형을 개발한 것으로 평가된다. 본 연구는 현재 한국의 건강보험 시범사업 수가체계 내에서도 방문진료의원이 운영 가능함을 보여주었다는 점에서 중요한 의의를갖는다. 향후 후속연구를 통해 방문진료의원의 확산모형 설계를 위한 종합적인 분석들이 심도있게 이루어져야 할 것이다.

영문 초록

This case study sought to explore whether home-based primary care can be an effective model for homebound older adults and people with disabilities in South Korea. The number of older adults with disabilities is expected to increase rapidly in the near future according to population aging trends. Thus, a care model in which doctors go directly to the patient’s home could address many complex medical needs and concerns. The Visiting Doctors Program of Medical Home was begun in March 2019 by two doctors and one nurse. Thus far, 2,070 home visits have been conducted to a total of 292 homebound older adults and people with disabilities. It was witnessed that there has been high patient satisfaction, reduced nursing home admission, development of various types of home healthcare services, establishment of a community care network, and support for in-home hospice care as achievements. The Visiting Doctors Program has developed a successful care model despite having a completely different operating structure from traditional outpatient clinics, with care only provided during in-home visits. The Visiting Doctors Program can be operated within the current health insurance fee system in South Korea. Comprehensive analyses and in-depth follow-up studies are needed to design disseminating models of home-based primary care.
