필라델피아 간편이름대기 평가의 번안 및 국내적용

국문 초록

성인 및 노인의 의사소통장애에 영향을 미치는 주요 요인들 중의 하나가 이름대기의 곤란증상이다. 인지언어기능 퇴행의 가장 보편적인 증상인 이름대기는 뇌손상으로 인한 언어능력 결함을 평가하는 중요한 도구이다. 하지만 안타깝게도 국내 성인 및 노인들의 이름대기 문제(naming problems)를 진단할 수 있는 평가도구는 매우 제한적이다. 이 예비연구는 국외 언어재활 현장에서 임상적 및 연구적으로 활용되고 있는 필라델피아 이름대기 평가(Philadelphia naming test, PNT)의 간편 버전을 국내 성인에게 적용하여 평가어휘의 이름대기 정확도, 어휘 친숙도와 사용빈도를 알아보았다. 대상자는 의사소통장애가 없는 한국어를 모국어로 사용하는 성인 70명(20대 45명, 60대 25명)이 참여하였다. 평가도구는 풀버전 PNT (175개 평가 그림과 목표 어휘)의 각 30개 어휘로 구성된 간편버전 PNT30-A와 PNT30-B를 실시하였다. 평가결과에 대해서는 20대와 60대 집단 내에서 두 개 평가도구간의 차이를 알아보기 위해 t-검정을 실시하였다. 연구결과 20대 집단에서는 두 개 평가도구 간의 정확도 점수, 어휘 친숙도, 사용 빈도에서 통계적으로 유의한 차이를 보였으나, 60대 집단에서는 유의한 차이가 나타나지 않았다. 두 개의 평가도구 모두에서 20대 집단의 이름대기 정확도 점수가 높게 산출되었다. 그리고 어휘 친숙도와 사용 빈도에서도 두 집단 간의 차이가 나타났다. 추후 국내 관련 영역의 표준화된 검사도구와의 상관성을 연구하여 국내 성인 환자 적용의 타당성과 신뢰성을 알아볼 필요가 있다.

영문 초록

One of the major factors affecting communication disorders in adults and the elderly-adults is word-finding difficulties. Naming problems, the most common symptom of cognitive language degeneracy, has an important role for assessing language ability defects caused by brain damage. Unfortunately, there are very limited evaluation tools for diagnosing naming problems in Korean adults and the elderly. This preliminary study applies a simplified version of the Philadelphia naming test (PNT), which is used clinically and research in foreign language rehabilitation sites, to adults in Korea. The subjects were 70 adults who spoke Korean as their native language. The evaluation tools used were simple versions PNT30-A and PNT30-B, which consisted of 30 test words of the full version PNT (175 evaluation pictures and target words). For the analysis of results, t-test was conducted to find the difference between the two evaluation tools in the 20 age and 60 age group. The results showed statistically significant differences in accuracy scores, words’familiarity, and frequency of use among the two evaluation tools in the 20 age group, but no significant differences were found in the three evaluation areas in the 60 age group. The naming accuracy scores of the 20 age group were higher in both evaluation tools than 60 age group. In addition, there was a difference between the two groups in terms of words’ familiarity and frequency of use. In the future, it is necessary to examine the validity and reliability of the application of aphasic adults in Korea by studying the correlation with standardized test tools.
