만성질환자의 소득수준에 따른 우울궤적 분석

국문 초록

본 연구는 만성질환을 가진 대상자들의 우울궤적을 고찰하고, 우울궤적 영향요인들이 무엇인지를 파악하고, 소득수준별로 우울궤적 예측요인이 다른지를 검증하는 연구이다. 이를 위해 한국복지패널의 1차-7차년도까지의 자료를 이용하여, 3개월 이상 만성질환으로 투약 및 투병의 형태로 치료를 지속하고 있는 2,423명을 대상으로 만성질환자들의 소득수준에 따른 우울궤적을 잠재성장모형과 다중집단분석을 이용하여 종단 분석하였다. 분석결과, 일반집단에 비해 저소득집단 만성질환자들의 우울수준이 뚜렷하게 더 높았고, 사회경제적으로 취약한 요소들에 더 많이 노출된 것으로 나타났다. 우울은 7년간 선형 관계를 보이며 점점 감소하였고, 집단간 뚜렷한 차이는 7년간의 긴 시간이 지남에도 여전히 저소득집단의 우울이 높은 양상이 그대로 지속되는 것으로 나타나는 정신적인 불평등 양상의 고착화 현상을 확인할 수 있었다. 소득수준별 다중집단분석 결과는 연령, 교육수준, 직업유무의 3개의 경로에서 차이가 나는 것으로 확인되었다. 이러한 결과를 바탕으로 만성질환자들의 우울궤적이 감소하는 현상과 소득수준에 따라 우울 차이가 발생하는 현상을 설명하고, 만성질환자들의 우울 개선을 위해 요구되는 실천적, 정책적인 개입방안을 논의하였다.

영문 초록

The present study is to investigate the trajectories of depressive subjects with chronic diseases and identify what are depressed trajectory Factors and validate predictive factors of depression trajectories differ by income level. To this end, 2,423 patients who were treated with chronic diseases for 3 months or longer using data from the 1st to 7th years of the Korean Welfare Panel were enrolled in this study. The depressed locus Were analyzed using latent growth curve model and multiple structural equation analysis. As a result, the depression level of the low - income group was significantly higher than that of the general group, and it was found to be more exposed to socioeconomically weak elements. The depressed group showed a linear relationship for seven years and decreased gradually. The significant difference between the groups was confirmed by the persistence of the mental inequality pattern in which depressive aspects of the low income group persisted even after a long period of 7 years. The results of multiple group analysis by income level showed that there were differences in age, education level, and occupation status. Based on these results, the depression trajectory of chronic illnesses decreased and the depression difference occurred according to income level, and the practical and policy interventions required for the improvement of depression of chronic illnesses were discussed.
