연명의료 환자에 대한 노년 남성의 배우자 돌봄: 문화기술지 연구

국문 초록

고령화와 가족 구조의 변화 속에서 노인 돌봄에서 배우자 돌봄이 차지하는 비율이 점차 증가해 왔으며, 이에 따라 노년 여성뿐 아니라 노년 남성의 배우자 돌봄에 관한 연구도 절실히 요구되어왔다. 이러한 상황 속에서, 이 연구는 문화기술지 방법을 사용하여 노년 남성의 배우자 돌봄이 이루어지는 맥락과 특성을 깊이 있게 이해하려는 목적으로 수행되었다. 특히 이 논문에서는 노년 남성이 수행하는 돌봄의 방식과 감정적 태도에 주목하면서, 노년 남성이 남편이자 아버지로서의 젠더정체성을 어떠한 역학적인 관계 속에서 실천하고, 또 돌봄을 둘러싼 다양한 의미의 경합을 만들어내는가를 분석하였다. 이 연구에 따르면, 남성 배우자가 아내의 연명 치료를 지속하도록 결정하는 데는 생애사적인 경험과 그러한 경험에서 비롯된 돌봄에 대한 심리적인 의미화가 깊이 관여하고 있었다. 환자의 돌봄에는 주요한 결정의 권한과 책임을 지고 있는 남성 배우자 외에도 자녀, 간병인, 의료진이 함께 ‘돌봄의 그물망(webs of care)’을 구성하고 있었으며, 연명의료의 특수한 상황 속에서 남성 배우자의 가부장적 권한과 자녀들의 희생, 그리고 거대한 비용 지출을 통해서만이 돌봄이 지속될 수 있었다. 연명의료가 장기화하는 상황 속에서 남성 배우자, 자녀, 간병인은 돌봄의 대상, 돌봄의 방식, 그리고 돌봄의 성격에 관해 서로 다른 입장을 드러내며 의미의 경합을 나타내기도 했다. 결론적으로 이 연구는 노년 남성의 배우자 돌봄이 그동안 환자와 맺어온 생애사적 경험, 돌봄수혜자의 의료적 상황, 그리고 ‘돌봄의 그물망’ 내 역학관계 속에서 복잡한 특성을 갖게 될 뿐 아니라, ‘무엇이 과연 환자에게 바람직한 돌봄인가’라는 근본적인 질문을 남기고 있음을 확인하였다.

영문 초록

Due to rapid population aging and the change in family structure, spousal caregiving has been becoming increasingly important in Korean elderly care, increasing the urgency of research in elderly men who care for their spouses as primary caregivers. Based on three months of ethnographic fieldwork in the hospital and the research participant’s home, this study examines the gender identity and psychological attitudes of a male caregiver who cares for his wife receiving life-sustaining treatment. This paper examines particularly on how the male caregiver’s gendered experience and embodied identity as the head of a family influence the ways he makes decisions on how his wife is taken care of. The paper also discusses the characteristics of his spousal care and what it implies in a broader cultural context. The research result shows that through his authority and leadership as a patriarch, the male caregiver has established “webs of care” by gathering his children, occupational caregivers, and a medical team. The prolonged life-sustaining treatment for his wife has been only possible in this system. Those who take part in the system, however, have not fully agreed with the male caregiver’s opinions, even while they have complied with his commands. Other participants’ different positions of care have gradually generated a competition in meaning around various aspects of care. In conclusion, this paper argues that the elderly man’s spousal care is a dynamic and complicated matter that is associated with lifelong experience with the patient, the care-receiver’s medical condition, and relationship among the members of the “webs of care.” It also leaves a fundamental question of what is ultimately the most desirable and ethically appropriate care for a patient receiving life-sustaining treatment.
