만성뇌졸중환자의 사회참여를 위한 재활중재

국문 초록

본 연구는 지역사회에 거주하고 있는 만성뇌졸중환자를 대상으로 재활중재를 적용한 무작위 대조 연구(Randomized Controlled Trials: RCT) 중 사회참여의 효과를 제시한 연구의 체계적 고찰 및 메타분석을 통해 만성뇌졸중환자의 사회참여를 위한 재활중재를 ICF 영역에 따라 분류하여 효과를 제시하고자 하였다. 본 연구는 2008년 1월부터 2018년 10월까지 국외에 게재된 연구로 PubMed MEDLINE, Cochrane CENTRAL, Google Scholar 데이터베이스를 사용하였고, 검색논문의 참고문헌을 추가하여 이차검색을 실시하였다. 각 연구의 질 평가는 PEDro Scale을 사용하였다. 포함 기준에 맞는 8편의 연구가 최종 선정되었고, 메타분석은 RevMan 5.3 소프트웨어를 사용하여 대상 논문의 실험군, 대조군의 대상자수, 평균, 표준편차값으로 통계적 이질성, 효과크기, 출판편의을 산출하였다. 그 결과 대상자는 총 903명이였고, 신체기능 및 구조의 효과크기는 .70으로 보통 효과크기(p>.05), 활동과 참여의 효과크기는 2.27로 큰 효과크기였다( p <.05). 본 연구의 결과를 통해 만성뇌졸중환자의 사회참여 중재에 신체 기능 및 구조에 초점을 맞춘 중재와 활동과 참여에 초점을 맞춘 중재 모두 효과적이고, 활동과 참여에 초점을 맞춘 중재가 좀 더 효과적임을 알수있었다. 본 연구를 기반으로 사회참여 증진을 위한 중재에 유용한 근거가 될 수 있을 것이다.

영문 초록

The purpose of this study was to investigate the systematic review and meta analysis of rehabilitation intervention for social participation of chronic stroke patients. This study used PubMed MEDLINE, Cochrane CENTRAL database from January 2008 to October 2018, and secondary search was performed by adding references of search papers.The quality of each study was assessed using the PEDro Scale. A total of 8 studies were selected for inclusion criteria and meta-analysis was performed with the RevMan 5.3 software using the experimental group, the control group, the mean, and the standard deviation. As a result, the number of subjects was 903, the effective scores for body function and structure was .70, which demonstrates medium effect size (p>.05) and the effect size of activity and participation were 2.27 , which demonstrates large effect size(p<.05). The results of this study showed that intervention focused on physical function and structure and intervention focused on activity and participation were effective and It was found that intervention focused on activities and participation was more effective in social participation intervention of chronic stroke patients. Based on this study, it can be a useful basis for intervention to promote social participation.
