국문 초록

본 연구의 목적은 자기보고건강과 우울증 경험여부를 통하여 지역별 소득불평등 정도를 비교･분석하는 것이며, 질병관리본부의 2013년 지역사회건강조사를 이용하여 지니계수와 유사한 집중지수(Concentrtion Index, CI)를 측정하였다. 집중지수는 간접표준화방식을 이용하여 연령과 성별을 표준화한 집중지수와 건강변수의 이원성을 반영할 수 있도록 보정된 Wagstaff 와 Erreygers의 집중지수를 계산하였다. 연구결과 저소득층에게 건강불평등이 집중되어 있음을 발견하였다. 자기보고건강(나쁨)의 집중지수는 –0.032(표준오차: 0.001), 우울감 경험의 집중지수는 –0.142(표준오차: 0.007)로 나타났다. 또한 소득관련불평등의 정도는 지역에 따라 다르게 나타났는데, 이는 특정 지역에서는 소득과 건강사이의 연관성이 더 강하다는 것을 의미하며 사회안전망과 건강형평성을 강화할 필요가 있음을 시사한다. 따라서 소득과 건강사이의 연관성을 강화시키는 지역적 요인을 확인하기 위한 후속연구가 필요하다.

영문 초록

The purpose of this study is to assess and compare the extent of income-related inequality in the prevalence of self-reported depression and ill-health across regions. Data were taken from the 2013 Community Health Survey (n=228,781). Income-related health inequality was measured using the concentration index(CI) which is a bivariate analog of the Gini coefficient. Age and sex-standardized CI was also calculated using an indirect standardization method. In addition, both Wagstaff and Erreygers CIs were calculated to reflect the binary nature of the health variables. The results generally indicated pro-rich inequalities across regions, which means that depression and ill-health were more concentrated on poorer individuals with the CI of –0.142(SE: 0.007) for depression and that of –0.032(SE: 0.001) for ill-health. In addition, the extent of income-related inequality varied across regions, implying that the link between income and health is stronger in certain regions. These findings suggest a need for strengthening the social safety net and health equity. Further research is also clearly needed to identify regional factors reinforcing the link between income and health.
