국문 초록

본 연구는 2017년도 노인실태조사 자료를 중심으로 노인의 일상생활능력이 건강만족도에 미치는 영향을 분석하였으며, 두 변수 간 관련성에 있어 우울증의 매개효과가 존재하는지를 분석하였다. 분석을 위해, 3개월 이상 요양원에 입소 혹은 요양병원에 입원한 경험이 있는 65세 이상 132명의 노인들을 대상으로 표본을 구성하였다. 연구결과는 다음과 같다. 첫째, ADL은 건강만족도에 직접적인 영향력이 없었다. 둘째, ADL은 우울증에 유의한 예측변인이었다. 셋째, ADL과 우울증 변수를 투입한 모델에서 우울증이 ADL과 건강만족도와의 관계를 유의하게 완전 매개하였다. 즉, ADL이 제한적일수록 우울증이 높았고, 우울증이 높을수록 건강만족도는 낮아지는 것으로 나타났다. 이러한 결과는, 요양시설 이용경험이 있는 노인에 있어서 ADL이 직접적으로 건강만족도에 영향을 주기보다는 우울증이 수반될 경우 건강만족도에 유의한 영향을 나타낸다는 것이다. 본 연구의 함의는 장기요양시설에 거주경험이 있는 대상자의 ADL과 건강만족도의 관계를 규명하고자 할 경우, 우울증 수준을 고려하여 모델에 설정하여야 한다는 점이다. 장기요양시설에서 입소자들의 정신 건강유지를 위해 우울증 완화 프로그램 등의 필요성에 대해 언급하였다.

영문 초록

This study investigated the effect of the ADL(Activities of Daily Living) on the health life satisfaction and the mediating role of depression between the ADL and health life satisfaction using the 2017 Elderly Living Conditions Survey. A sample of 132 individuals 65 or older was adopted, and they all have experienced in staying in long-term care hospitals for a duration of three months or more. It was hypothesized that the level of depression mediates the relation of the ADL with the health life satisfaction. The study results were as follows. First, the ADL did not have a direct effect on the health life satisfaction. Second, the ADL was a significant predictor of depression. Third, the depression acted as the complete mediation factor for predicting the health life satisfaction when both the ADL and depression was entered in the final model. That is, when the ADL was more limited, the depression was higher. In turn, the higher depression led to have the lower likelihood of the health life satisfaction. Therefore, although the ADL did not show a direct effect on the health life satisfaction, the ADL had an indirect effect with depression on the health life satisfaction. This result suggested that depression should be considered as a mediating factor for the ADL in case of modeling the relationship between the ADL and the health life satisfaction. For the maintenance of better mental health for the residents of long-term care facilities, the implementation of depression-alleviating program was suggested.
