국문 초록

이 연구는 Gelberg-Andersen 모형을 적용하여 남한에 거주하고 있는 북한이탈주민의 건강정보이해능력에 영향을 미치는 요인을 모색하고자 하였다. 조사대상은 2015년 현재 만20세 이상 북한이탈주민으로 대구, 울산, 부산광역시에 거주하고 있는 자를 편의표집하여 Chew의 16문항 HL 척도를 포함한 구조화된 설문지로 면접조사를 시행하여, 최종 228부를 분석하였다. 조사결과, 북한이탈주민의 건강정보이해력은 5점 척도 기준, 2.77 이었고, 이를 선행연구(손연정·송은경, 2012)의 연구결과에 조응시켜 해석하면, 적절-경계-부적절수준 영역구분에서 경계범위 위치함을 발견하였다. 회귀분석결과 이러한 북한이탈주민의 HL 수준에 영향을 미치는 요소로서, 성별, 북송경험, 사회적지지, 입국 후 종교유무, 30분이상의 주별 운동횟수 등이 유의미하게 나타났다. 전체적으로 북한이탈주민의 건강정보이해능력의 결정을 설명하는 것으로서의 Gelberg-Andersen모형은 강력하게 지지되기 어려운 것으로 나타났다. 특히 ‘욕구요인(needs factors) 에서의 취약계층영역 변인의 영향력은 이 연구에서는 탐지되지 않아서 북한이탈주민들의 경험에 관련된 추가적인 변인들이 발굴, 동원될 필요가 있다. 조사결과를 기반으로 연구의 이론적, 실천적 함의가 논의되고 제시되었다.

영문 초록

This study aims to search for the significant factors associated with health literacy(HL) among North Korean defectors(NKDs) who are resettling in the South Korea, under the theoretical approach of Gelberg-Andersen Behavioral Model. Research subjects were selected among the NKDs aged over 20 and living in Daegu, Ulsan, and Busan metropolitan city. They were asked to answer for the quesionnaire including the 16 items of Chew scale for HL, and finally 228 cases were analyzed for this study. Average of HL among the subjects is 2.77(sd.= .62) which locates in the level of wanring , meaning a little bit higher than the level of inappropriate , but lower than appropriate applying the scoring tool of a prior study(Son and Song, 2012). Regression analysis finds that ’social support recognized by the subjects and frequency of physical exercise over 30 minutes within a week have a positive association with HL. Other significant factors such as gender, post-entry religion effect, and repatriation experience were not easy to be interpretated in the association with HL, although their strengths of impact on HL are not weak. Overall, the Gelberg-Andersen approach seems not to be strong to explain the variation of HL among NKDs resettling in South Korean society. In particular, additional and unique variables in the domain of needs factors need to be searched more and testified in their influence on the variation in HL of NKDs. Theoretical and practice implications follow the research findings and discussion.
