국문 초록

이 논문은 충격적 사건 중 특히 비윤리적인 일을 경험한 후 분노, 슬픔, 자기비하, 죄책감, 수치심과 같은 감정에서 벗어나지 못하는 상태인 도덕적손상(moral injury)이라는 개념을 소개하고 국내 정신건강증진 및 지역사회 복지실천에의 적용가능성을 고찰하는데 그 목적이 있다. 도덕적손상이란 전쟁 참전용사들이 지역사회로의 복귀 후 경험하는 다양한 유형의 부적응 및 디스트레스 상황에 대해 PTSD라는 정신 병리적 진단과 개입방법을 가지고 접근하는 것이 탈-맥락적임을 인식하면서부터 미국에서 처음 제안된 개념이다. 최근 국외에서는 참전용사들 외에도, 그들의 가족, 자살한 부모의 자녀들, 윤리적 딜레마를 자주 경험하는 학교선생님 등, 정의롭지 않은 일 혹은 비윤리적이거나 부도덕한 일을 경험한 후 현실에 적응하지 못하는 현상을 이해하고 개입하는데 도덕적손상을 적용하는 시도가 늘고 있다. 이에 본 연구는 국외연구문헌을 고찰한 후 최근 우리나라에 발생한 세월호 침몰사고가 범국민적으로 미친 심리적인 영향을 이해하는데 도덕적손상의 개념 적용을 시도해 보았다. 부도덕한 일을 직간접적으로 경험한 이후에 발생하는 부정적 정동 상태에 대한 개인적 정신 병리로의 접근에서 벗어나고자 하는 도덕적손상이라는 개념은 최근 우리 모두가 목격한 세월호 침몰사건이나 정부와 기업의 부정부패에 대한 정보를 접한 후 유독 일상으로의 적응을 힘들어하는 국민들을 위한 지역사회기반 정신건강증진 방안을 고민하는데 새로운 관점을 제시할 수 있을 것이다.

영문 초록

The purpose of this study is to introduce the concept of Moral Injury as being stuck in the distressful responses such as embitterment, sorrow, self-degrading, guilt, and shame, after experiencing traumatic-unethical events and to explore the applicability of this concept in the context of Korean community mental health and social work practice. Moral Injury was recently suggested in the United States after recognizing how out-of-context and overly psycho-pathological PTSD was in understanding various post-traumatic distresses among veterans. Recently, studies published in English started to apply the concept of Moral Injury in understanding the experiences of veterans, family members of veterans, survivors of parents’ suicide, school teachers who often suffer from ethical dilemma, and those experiencing other types of unethical events or injustice. Thus, after reviewing literature on moral injury, this study attempts to re-examine the results of studies published in Korean that discussed negative psychological effects of Sewol Ferry Disaster by applying the concept of Moral Injury. The concept of moral injury as an attempt to approach distressful responses after experiencing immoral events moves us away from psycho-pathological and out-of-context perspective. Moral injury might provide an idea for a community-based-mental-health-promotion approach in helping those having adjustment difficulties after witnessing Sewol Ferry Disaster and government and business corruptions.
