편두통의 형태에 따른 Topiramate의 치료효과 비교

국문 초록

목적: 편두통 증세에 대한 topiramate의 효과를 분석함으로써 편두통의 형태에 따른 특이적인 효과가 있는지 알아보고자 하였다. 방법: 2006년 1월 1일부터 2013년 6월 30일까지 삼성창원병원 소아청소년과에서 만성 편두통으로 처음 진단되었던 소아청소년들 중 진료 종료 후 최소 1년간의 경과가 완전히 파악된 73명을 대상으로 하였다. 모든 환자들에게 완화요법에 의한 치료를 시행함과 동시에 topiramate 12.5 mg 하루 2회 복용에서 시작하여 최대 하루 100 mg 까지 증량하며 편두통의 경과를 관찰하였다. 무조짐 편두통과 조짐 편두통으로 이분하여 치료 종료 후 1년까지의 경과를 분석하여 topiramate가 편두통에서 형태-특이적인 효력이 있는지 확인하였다. 결과: 치료 후 최소 1년간 조짐 편두통에서 67.4%에서 쾌유하였고 무조짐 편두통은 33.3%가 쾌유한 것으로 나왔다( P =0.004). Topiramate의 용량은 무조짐 편두통 환자가 50.0±23.6 mg, 조짐 편 두통 환자가 30.2±10.3 mg 으로 조짐 편두통 환자들이 완쾌를 위해 저 용량의 topiramate를 복용하였다( P =0.001). 치료 전 무조짐 편두 통 및 조짐 편두통 환자들의 평균 월 두통 횟수는 각각 22.4±5.2회, 21.4±5.6회였으며 치료가 종료된 후에는 각각 3.9±6.3회, 1.3±3.4회 로 양측 모두 두통의 횟수가 감소하였으며 그 중에서도 조짐 편두통 군에서의 월간 두통의 횟수가 더욱 감소하였다( P =0.046). 또한 쾌유 군 에서 무조짐 편두통의 경우 27.0±25.5일, 조짐 편두통의 경우 9.6 ±5.2일로 조짐 편두통 환자들에서 두통완화에 소요되는 시간이 짧았고( P =0.001) 쾌유군 및 호전군 에서 나이와 topiramate 복용 용량과는 상관관계가 없었다( r =-0.098, P =0.554). 결론: Topiramate를 치료제로 사용한 동일한 치료환경에서 치료한 만성편두통 환자 중 조짐 편두통 환자에서 증세가 조절될 가능성이 높고 조짐 편두통 환자 군에서 저용량으로도 보다 잘 조절되며 회복 시점도 빨라 조짐 편두통의 치료에 topiramate가 특이적인 것으로 사 료된다. 또한 조짐 편두통의 치료 용량은 환자의 나이와 몸무게와 무관한 것으로 사료된다.

영문 초록

Purpose: The purpose of this study was to evaluate a specific effect of topiramate in accordance to the type of migraine. Method: A total of 73 children and adolescents who had newly diagnosed chronic migraine and followed up for more than 1 year after the termination of treatment were involved in the study. All patients were treated by taking topiramate and intervention of behavior therapies simultaneously at first. Start dose of topiramate was 25 mg/day and the dose was increased to 100 mg/day, according to patient's need. We examined whether topiramate has the type-specific effect or not by analysing of the change of number of migraine attacks for more than 1 year after termination of treatment in migraine without aura (MOA) group and migraine with aura (MA) group. Results: 67.4% of patients of MA and 33.3% of patients of MOA were cured(P=0.004). Average dosage of topiramate were 50.0±23.6 in MOA group and 30.2±10.3 mg in MA group. Therefore the patients with MA took less dose of topiramate than the patients with MOA (P=0.001). Average frequency of migraine attack were 22.4±5.2/month in MOA group and 21.4±5.6 in MA before treatment. But the frequency was markedly decreased as 3.9±6.3/month in MOA group and 1.3±3.4/month respectively(P=0.046). The taking time to improvement were 22.0±19.3 days in MOA group and 9.5±4.8 days in MA group, therefore, the time to be required for improvement was shorter in the group of MA than MOA in patients of the cured and the partially cured(P=0.001, 0.017, respectively). No correlation was observed between patients' age and topiramate dosage in the cured and partially cured group(r=-0.098, P=0.554). Conclusion: The possibility of improvement was higher in the patients of MA than in MOA. The patients with MA took less dose of topiramate than MOA. The frequency of migraine attack was decreased more in the group of MA. And the time to be required for improvement was shorter in the group of MA than in MOA. Therefore all results taken together, it is concluded that topiramate has specific effect for MA.
