Factors Affecting Life Satisfaction and Depression of Caregivers of the Elderly with Dementia: A Comparative Study of Korean and American Caregivers

국문 초록

본 연구에서는 한국과 미국의 치매노인 가족보호자의 생활만족과 우울에 미치는 영향요인을 비교분석하였다. 병약한 노인인구가 급속하게 증가하고 있는 한국에서 가족보호자들을 위한 제도를 수립하기 위해서는 그들의 관한 더 많은 정보가 필요하다. 본 조사는 한국의 3개 도시(서울, 광주, 부산)에서 5세 이상의 치매노인을 돌보고 있는 가족보호자들을 대상으로 실시되었다. 한국에서 290명의 가족보호자들을 대상으로 한 분석결과는 123명의 가족보호자를 대상으로 한 미국의 연구결과와 비교되었다. 한미비교의 결과에서는 몇 가지 유사점과 상이점이 발견되었다. 사회적 지지와 활동, 회피적 대처, 그리고 수발상황에 대한 스트레스적 평가 등은 가족보호자들의 생활만족도를 감소시키고, 우울증의 정도를 증가시키는 경향을 공통으로 보여주었다. 반면, 한국의 가족보호자들은 미국보호자들에 비해, 대처기술이 적게 사용하고 수발상황에 대해 부담을 표현하는데 절제하는 모습을 보여주었다. 특히 한국보호자들은 적극적 대처기술을 거의 사용하지 않으며, 그 대신 사회적 지지망에 더욱 의존하는 경향을 보였다. 구체적으로 한국에서 경로분석의 결과를 보면 가족 보호자의 소득과 건강상태가 더 좋고, 사회활동을 더 많이 하며, 노인보호에 대해 적극적인 극복기술을 더 많이 사용하고, 소극적인 극복기술을 덜 사용할수록 자신의 생활전반에 대한 만족도가 더 높은 것으로 나타났다. 반면 가족보호자들의 건강이 좋지 않고, 스트레스원이 증가할수록 생활만족도는 낮아지고, 소극적인 극복기술을 더 많이 사용할수록 더욱 우울한 경향을 나타냈다. 본 연구결과는 가족보호자가 사회적으로 취약한 계층에 속할수록 그들의 심리적 복지는 심각한 위기를 겪게 된다는 점을 보여준다. 그러므로 이들을 위한 사회적 개입과 대책이 절실하다는 점을 알 수 있다. 오늘날 한국에서는 자녀의 부모부양의식이나 가족의 책임이라는 가치가 점차 쇠퇴하고 있으므로, 가족보호자들에게 사회적 차원의 공식적인 지지와 개입을 통한 노인보호를 원활하게 해줄 수 있는 방안이 마련되어야 할 것이다.

영문 초록

This study presents the factors affecting on Satisfaction, Depression of Caregivers for the Dementia elderly across South Korea and America. The purpose of this study is to find out how mediators buffer the advase effects of caregiving situations, finally improve caregiver well-being. To catch differences in the mediating roles between the two countries is another purpose. The survey was conducted with caregivers of d e m t i a patients aged over 65 in Korea. Demographic and Survey result of 290 caregivers were compared with the result of 123 -can caregivers. In the companson several differences and similarities were found. From compamtive analyses, Korean caregivers were found to be in more adverse conditions and experience more burden related to caregiving. In the path analysis, Korean caregivers with higher social status, better health, more social activity, and less we of avoidance coping were more likely to show higher life satisfaction and lower depression But, caregivers With poor health, more stressful appraisal, and m ore of avoidance coping were more likely to show laver level of well-being. these results were similar to the American results. However, Korean caregivers were less likely to we approach coping strategies, to be expressive about stressful caregiving condition than American ones. Instead, they tend to depend on informal support networks. Currently in Korea, the strong value of filial piety and family obligations are decayed. Given these social changes, it is essential to consider public policies and intervention both at micro and micro level for caregivers.
