노인요양시설 위험사고 발생가능성에 영향을 미치는 요인에 관한 탐색적 연구

국문 초록

본 연구는 노인요양시설의 시설 특성을 비롯한 시설의 위험 정도와 위험관리 수준 및 필요성에 대한 개인별인식수준을 파악하고 유형별 위험사고 발생 가능성에 영향을 미치는 요인을 탐색하여 위험 관리방안을 제안하는데 그 목적이 있다. 이를 위해 부산지역 노인요양시설 89개소 각 시설별 최장근무 요양보호사 1명을 대상으로 구조화된 설문지를 활용하여 종사자 인식 조사를 실시하였다. 응답시설 72개소 중 통계처리가 불가한 2개소를 제외하여 최종 70개소의 설문지를 SPSS 18.0 통계프로그램을 활용하여 기술통계(Descriptive Analysis)와 위계적 회귀분석(hierarchial regression analysis)을 실시하였다. 조사결과, 위험유형별 발생가능성(5점 척도)은 업무위험 1.9, 사회적 위험 2.1, 재해위험 2.2, 경영(운영)위험 2.7로 나타나 요양보호사들은 경영위험을 가장 높게 인식하고 있었다. 각각의 위험사고 발생가능성에 영향을 미치는 요인으로는 시설 위험 수준(β=.422, p=.001)과 시설 위험관리 수준(β=.289, p=.013)이 정(+)의 영향을 미치는 것으로 나타났다. 사회적 위험 발생가능성에 유의한 영향을 미치는 요인으로는 시설의 위험관리 수준(β=-.355, p=.006)이 p<.01 수준으로 나타났으며, 재해위험 발생가능성에 유의한 영향을 미치는 요인으로는 시설 운영주체(β=.286, p=.031)가 정(+)의 영향력을 미치는 것으로 나타났다. 연구결과를 중심으로 노인요양시설의 위험관리 방안을 제언하면, 조직적 요인으로서 시설운영주체를 비롯한 시설의 물리적 환경의 위험사고 정도를 파악할 수 있는 실태조사와 이와 관련한 법령이나 지방자치단체 조례의 제정비가 필요하다. 그리고 개인적 요인으로서 서비스 제공자의 책임성을 강화 할 수 있는 위험관리에 대한 인식제고와 위험관리매니저 임명, 위험관리 교육 및 사례발표의 의무화가 필요하다. 특히 노인요양시설의 경우 입소자 특성상 위험사고의 발생 가능성이 높고, 사고에 따른 피해 정도가 심각하기 때문에 향후 지속적이고 체계적인 위험관리시스템이 구축되어져야 할 것이다.

영문 초록

This study puts its purpose in exploring the factors, affecting the occurrence possibility of risk incident and suggesting plans to preliminarily prevent and manage the risks which can be likely to happen in senior nursing homes by identifying the actual conditions of risk incident and risk management as well as organizational characteristics. To achieve the study goal, it set caregivers of nursing homes as targets by choosing one who had the longest working experience in each facility of 89 senior nursing homes respectively, and it conducted questionnaire surveys on the actual conditions of the occurrence of risk incident within the facilities, risk management level, the occurrence possibility of risk incident and influential factors. 2 facilities were omitted among 72 respondents due to the problem of statistical processing. Finally, descriptive analysis and hierarchial multiple regression analysis were performed by using the SPSS 18.0 statistical program for questionnaire papers. The study results demonstrate that the occurrence possibility of risk types (5-point scale) was 1.9 in work risk, 2.1 in social risk, 2.2 in disaster risk, and 2.7 management (operation) risk. The factors affecting the occurrence possibility of each risk incident were facility risk level (β=.422, p=.001) and facility risk management level (β=.289, p=.013). These factors had a positive effect on the occurrence possibility with the level of p<.001 and p<.05 respectively. In view of the characteristics of senior nursing homes, the occurrence possibility of risk incident is high and the degree of damage caused by accidents is serious. Therefore, continual and systematic risk management is necessary. This study suggests some risk management plans of senior nursing homes based on the study results as follows: First, the survey of actual conditions and environmental improvement should be complemented. Second, mandatory education of risk management, which can strengthen the responsibility of service providers as well as the central operating body, is required. In this regard, the enactment of relevant legislation and local ordinances which can make the matters of risk management education systematic and mandatory is needed.
