뇌간병변 뇌졸중 환자의 병변에 따른 지표별 삼킴기능 비교

국문 초록

배경 및 목적: 본 연구에서는 삼킴장애를 보이는 뇌간병변 뇌졸중 환자들을 대상으로, 삼킴기능을 평가하는 여러가지 지표에서 병변에 따라 어떠한 양상이 나타나는지 비교하고자 하였다. 방법: 36명의 뇌간병변 뇌졸중 환자들을 병변에 따라 중뇌, 뇌교, 연수 병변으로 구분하였고, 10명의 일반인을 정상대조군으로 삼았다. 삼킴기능 지표로서 인두단계의 정량적 측정을 위해 설골의 최대 위쪽 이동거리와, STD, PTT를 측정하였고, 인두잔여물 정도 확인을 위해 인두연동작용과 윤상인두근 이완 여부를 살펴보았다. 그리고 침습-흡인 척도(PAS)를 통해 흡인 여부를 파악하였으며, ASHA NOMS를 이용하여 임상적 중증도를 나누었다. 결과: 15㏄ 묽은 액체에서의 STD와 PTT에서 6% 반고형식 15㏄와 묽은 액체 5㏄에서의 PTT를 제외한 모든 지표에서 통계학적 차이를 보이지 않았다. 논의 및 결론: 다양한 지표에서 살펴본 삼킴 기능은 병변에 따라 대부분 차이가 나지 않았다. 이는 삼킴과정과 관련된 해부학적 병변을 알아보는 연구를 함에 있어 뇌간을 해부학적 구조에 따라 크게 뇌교, 연수로 분류하여 비교하여 알아보는 것은 한계가 있다는 것을 시사한다.

영문 초록

Background & Objectives: The objectives of this study was to investigate the aspects of swallowing functions at the pharyngeal stage and the clinical severity according to the anatomical structures. Method: Subjects of the study included 36 patients with brainstem lesion and 10 normal control, and 7 indices were measured; pharyngeal peristalsis, cricopharyngeal functions, and presence of aspiration, the vertical hyoid bone movement, the stage transition duration(STD), the pharyngeal transit time(PTT), and clinical severity using the ASHA NOMS. Results: As a result, all most indexes except some indices(stage transition duration with 15㏄ thin liquid, pharyngeal transit time with 15㏄ semi-solid and 5㏄ thin liquid), there is no significant difference. Discussion & Conclusion: Almost swallowing functions shown by the various indicators did not differ from the lesions. This implicates that subdividing brainstem into the three sections (midbrain, pons and medulla) when studying the anatomical correlations with the swallowing process has limitation.
