자폐증 아동의 조기인식 요인

국문 초록

자폐증을 조기에 인식하고 정확하게 진단내리는 것은 치료와 향후 예후에 결정적인 요인으로 작용하며, 가족들의 적응과 대처방법 등에도 영향을 미쳐서 매우 중요하다. 이에 본 연구에서는 자폐증 아동과 정신지체 아동을 대상으로 하여 사회인구학적 특성, 가정환경, 임신 및 출산시의 문제, 자폐증의 임상양상, 발달영역별 기능수준의 비교를 통해 조기인식의 요인을 파악하고자 하였다. 부모가 아동이 문제가 있다고 처음 인식하게 된 시기, 자폐증평정척도, 산모의 늦은 나이, 저체중, 타인에 대한 인식, 사회적 놀이, 비언어적 의사소통, 상상력이 필요한 놀이, 상동적인 신체동작, 물건의 어떤 부분에 대한 지속적인 집착, 제한된 관심의 영역, 지각 영역과 대근육영역, 인지언어능력에서 의미있는 차이가 있었다. 따라서, 본 연구결과 기존 진단기준에는 어린나이의 아동에게 부적합한 임상증상 항목들이 존재한다는 것을 알게되었으며, 이 경우 별개의 진단기준이 필요하다는 것을 제안한다.

영문 초록

The diagnosis of autism or early- onset pervasive developmental disorder in very young children is difficult for both families and clinicians. This study is to facilitate early recognition of autism by identifying sociodemographic characteristics and family environment , prenatal and perinatal factor s on mother s, and behavior characteristics that are most apparent at young ages. Social interaction, communication, restricted activities and interest s were evaluated using standard diagnostic criteria for autism. Result s suggested that the areas of lack awareness of others, social play, nonverbal communication, imaginative play, stereotyped body movement s, part s of object s and range of interest s are more prominent than young child with mental retardation. Also, birth order , age on recognition, score of childhood autism rating scale, maternal age, birth weight and part of Psycho-Educational Profile appear to detect different aspect s of characteristics in this sample. In this study, a few symptom variables may represent on conver sation skill, speech production and content . This result suggest s that different criteria of autistic symptoms are recommend on recognition of autism in early age.
