국문 초록

클라리넷 연주자의 앙부슈어는 색소폰과 유사한 single-reed embouchure에 해당하며, 하순을 하악 전치에 위치시키고 상악 전치와 함께 마우스피스를 물게 되는 기본 구조를 가진다. 클라리넷 연주자가 상악 중절치의 완전 탈구 후 재식술을 받는 경우, 치아의 회복 기간 동안 심각한 기능적 제한과 합병증 발생의 위험에 직면하게 된다. 재식된 상악 중절치는 앙부슈어 형성에 핵심적인 역할을 하지만, 초기 회복 기간 동안에는 과도한 압력이나 진동에 노출될 경우 치아의 안정성과 예후에 부정적 영향을 미칠 수 있다. 이는 연주자의 기량 유지와 향상에 지장을 줄 수 있으며, 장기적인 경력 단절로 이어질 위험이 있다. 본 증례보고는 이러한 상황에서 연주자의 경력 단절을 방지하고 재식된 치아의 성공적인 회복을 돕기 위한 대안으로 치아의 회복 기간 동안 활용 가능한 맞춤형 앙부슈어 보조도구의 적용에 대한 증례이다. 이 보조도구는 재식된 치아에 가해지는 압력과 진동을 분산시켜 치아를 보호하면서도 정상적인 앙부슈어 형성을 가능케 하여, 연주자가 회복 기간 동안에도 연주 기술을 유지하고 발전시킬 수 있도록 지원한다. 10년 간의 경과관찰을 통해 클라리넷 연주자는 합병증의 위험이 높은 초기 회복 기간 동안 연주를 지속할 수 있었고, 특별한 합병증 없이 성공적으로 회복기를 보낸 후 정상적인 연주 생활로 복귀한 사례를 보고한다. 이러한 접근은 적절한 응급처치와 의료인의 음악가에 대한 이해로 재식된 치아의 안정적인 회복을 도모하면서도 연주자의 경력 단절을 효과적으로 예방할 수 있음을 시사한다. 본 증례는 치과 치료와 음악 연주 사이의 균형을 맞추는 방법론을 제시함으로써 전문 연주자들의 치과 치료 후 회복 과정에 시사점을 제공한다.

영문 초록

The embouchure of a clarinet player, similar to that of a saxophone, employs a single-reed embouchure, where the lower lip is positioned on the lower incisors and the mouthpiece is held between the upper incisors. When the player undergoes replantation surgery following complete avulsion of a maxillary central incisor, they face significant functional limitations and risk of complications during the tooth's recovery period. While the replanted incisor plays a crucial role in forming the embouchure, exposure to excessive pressure or vibration during the initial recovery period can negatively impact the tooth's stability and prognosis. This can hinder the player's ability to maintain and improve their skills, potentially leading to long-term career disruption. This case report presents the application of a custom-made embouchure aid as an alternative solution to prevent career interruption and assist in the successful recovery of the replanted tooth. The aid is designed to distribute pressure and vibration away from the replanted tooth while allowing normal embouchure formation, enabling the player to maintain and develop their playing technique during the recovery period. Through a 10-year follow-up, we report on a clarinet player who was able to continue playing during the high-risk initial recovery period, successfully completed the recovery without significant complications, and returned to normal playing activities. This approach suggests that appropriate emergency treatment and a dentist's understanding of musicians' needs can promote stable recovery of replanted teeth while effectively preventing career disruption for players. The case provides insights into balancing dental treatment and musical performance, offering implications for the rehabilitation process of professional players following dental procedures.
