Gummy smile과 하악 양측 대구치 매복을 동반한 골격성 II급 부정교합의 치험례

국문 초록

웃을 때 과도한 치은이 노출되는 gummy smile은 심미적으로 바람직하지 못하며, 그 원인으로는 과도한 상악의 수직적 성장, 짧은 상순, 치관의 부분적 노출 등이 있다. 치료 방법으로는 교정적 방법과 수술적 방법이 있으며 그 원인과 심도를 고려해 바람직한 방법을 선택해야 한다. 하악 제2대구치 매복의 주요 원인으로는 인접치의 위치 이상, 공간 부족, 과잉치 등이 있다. 치료계획 수립 시 매복된 제2대구치의 경사도, 매복된 깊이, 제3대구치의 위치, 총생 등을 고려해야 하며 외과적 노출, 외과적 탈구를 통한 치아 재위치, 교정적 정출 등의 치료방법 중 적합한 것을 선택해야 한다. 본 증례에서는 gummy smile과 양측 하악 제2대구치의 매복을 동반한 환자에서 미니스크류를 통한 전치부 압하를 시행하여 심미적인 미소를 얻었고, 교정적 직립을 통해 매복 대구치를 견인하여 양호한 치료 결과를 얻었기에 이를 소개하고자 한다.

영문 초록

Gummy smile which is an excessive exposure of gingiva when laughing, is aesthetically undesirable. Causes of the gummy smile include excessive vertical growth of the maxilla, short upper lip, and partial exposure of the crown. Treatment methods include orthodontic and surgical methods, and the preferred method should be selected considering its cause and severity. The main causes of mandibular second molar impaction are abnormal position of adjacent teeth, crowding, and supernumerary teeth. When planning the treatment plan, inclination of the impacted second molar, depth of the impaction, location of the third molar, and crowding should be considered. Treatment option for impacted molar include extraction, surgical repositioning, and orthodontic uprighting. Among these methods, the most desirable method should be selected considering various factors. In this case, we present a patient with a gummy smile and bilateral impacted mandibular second molar who was treated with upper anterior teeth intrusion by using miniscrew and orthodontic uprighting of impacted second molar to induce alveolar bone regeneration.
