국문 초록

입술 돌출의 해소를 위해서는 일반적으로 소구치 발치를 동반한 전치부의 후방견인이 치료 방법으로 시행되고 있다. 하지만 일부 환자들의 경우 소구치의 발치에 대한 거부감을 호소하기도 하며, 발치 공간 폐쇄 도중 심한 전치부 치근흡수 또는 치주상태 악화 시에는 잔존한 발치 공간 폐쇄에 어려움이 존재할 수 있다. 골격성 고정원이 소개된 이후 많은 종류의 치아 이동이 환자의 협조도 없이 가능해졌으며, 치아 이동의 한계를 극복하게 하였다. 안정적인 골격성 고정원의 사용을 통한 전체치열의 원심 이동은 효과적인 치료 방법으로 보고되었으며, 치열의 수직적 조절에도 적용할 수 있다. 본 증례에서는 입술 돌출을 가진 장안모 경향의 골격성 I급 부정교합 환자를 골격성 고정원을 이용한 전체 치열의 원심이동의 설측교정을 통해 안모 심미성을 개선하였기에 이를 소개하고자 한다.

영문 초록

For lip protrusion treatment, anterior teeth retraction with the extraction of the premolar is generally performed as a treatment method. However, some patients do not want to extract the premolars. There may be also difficulty in closure of the remaining extraction space when severe root resorption proceeds or periodontal conditions deteriorate. Since the introduction of orthodontic miniscrews, limitation of tooth movement has been overcomed in orthodontic treatment and various tooth movements have become possible without patient cooperation. Total arch retraction by the use of a stable skeletal anchorage system has been reported as an effective treatment method and can be applied to vertical control of the dentition. This case report shows a skeletal Class I malocclusion patient with a hyperdivergent profile and lip protrusion treated by lingual orthodontics and total arch retraction using a skeletal anchorage system which resulted in improvement of facial esthetics.
