정신 장애인을 위한 지역사회기반의 작업치료에 대한 체계적 고찰

국문 초록

목적 : 본 연구는 정신 장애인을 위한 지역사회기반의 작업치료의 중재내용 및 방법을 분석하여 정신건강 작업치료의 임상실행을 위한 근거를 제시하고자 한다. 연구방법 : 2010년부터 2020년까지 전자데이터 베이스인 Google Scholar, Web of Science, PsycINFO 를 통하여 논문을 검색하였다. 검색용어는 Community based occupational therapy AND Severe mental illness OR Psychiatric disability OR Mental disorder 를 사용하였다. 배제기준을 통하여 최 종적으로 15개의 연구를 분석대상으로 선정하였다. 결과 : 총 15편 연구들의 근거수준은 Level Ⅰ와 Level 2 가 대부분 차지하였고 연구대상군은 조현병이 가장 많이 차지하였다. 지역사회 기반의 정신작업치료는 작업기반의 중재 및 평가가 가장 많이 적용되었 다. 중재는 수단적 일상생활활동, 일 또는 사회참여 순으로 적용되었고. 중재결과를 도출하기 위해 작업 기반, 수행중심의 평가도구 외에도 삶의 질, 심리사회적 기능, 정신적 증상을 평가가 적용되었다. 결론 : 본 연구를 통해 정신건강작업치료사들의 근거기반 실행을 위한 기초자료를 제시하고 지역사회 정신 재활서비스에서의 작업치료의 실행영역을 살펴봤다는 점에서 의의가 있다. 추후에는 국내에서도 작업기 반의 중재를 적용한 지역사회 정신작업치료 실행이 본격화되어야 할 것이고 이에 관련된 다양한 연구가 실행되어야 할 것이다.

영문 초록

Objective: The purpose of this study is to examine a type of intervention, i.e., an evaluation tool applied to community-based occupational therapy for people with mental illness, as an evidence-based practice. Methods: For the main keywords of a database search, ‘Community based occupational therapy’ AND ‘Severe mental illness’ OR ‘Psychiatric disability’ OR ‘Mental disorder’ were used. Using Google Scholar, Web of Science, and PsycINFO, we systematically examined papers published in journals from 2010 to 2020. Finally, 15 studies were selected through exclusion criteria. Results: Most of the selected studies were focused on evidence levels Ⅰ and Ⅱ. Most studies applied to community-based occupational therapy intervention for people with mental illness were occupation-based intervention, including IADL, Work, and Social participation. Occupation- and performance-based evaluation tools, as well as tools used to evaluate the quality of life, psychosocial function, and psychiatric symptom, were applied. Conclusion: Through this systematic review, clinical evidence on community-based occupational therapy was provided for people with mental illness to the occupational therapist. Because community-based occupational therapy practice for people with mental illness is expected to be conducted within the country, related studies in this area are needed.
