초기 뇌졸중 환자의 휠체어 기술 훈련 프로그램의 효과: 무작위 대조 예비 연구

국문 초록

목적 : 본 연구는 초기 뇌졸중 환자에서 휠체어 기술 훈련 프로그램 적용이 휠체어 기술 능력, 휠체어 추 진 속도, 일상생활수행력에 미치는 효과를 알아보고자 실시하였다. 연구방법 : 초기 뇌졸중 환자 18명을 실험군과 대조군으로 무작위 배정하였다. 두 군 모두 일반적인 재활치료를 실시하였으며, 추가적으로 실험군에는 휠체어 기술 훈련을, 대조군에는 기립기를 통한 서기 훈련을 각 1일 30분, 주 3회, 총 3주간 실시하였다. 중재 전·후 휠체어 기술 능력을 측정하기 위해 휠체어 기술 검사 (Wheelchair Skills Test; WST), 휠체어 추진 속도(Wheelchair Propulsion Velocity; WPV), 한국판 수정 된 바델 지수(Korean version of Modified Barthel Index; K-MBI)을 사용하였다. 결과 : 3주 중재 후, 두 그룹에서 중재 전·후 WST, WPV, K-MBI에서 유의한 차이가 나타났으며(p < .05), 중재 후 두 그룹 간 변화량의 차이는 WST와 WPV에서 유의한 차이가 나타났다(p < .05). 결론 : 본 연구 결과 휠체어 기술 훈련 프로그램이 초기 뇌줄중 환자의 휠체어 기술 능력과 휠체어 추진 속도를 향상 시킬 수 있는 중재 방법으로 나타났다. 그러므로 휠체어 기술 훈련 프로그램은 독립적인 독 립적 보행이 불가한 초기 뇌졸중 환자에게 안전하고 효율적인 이동 능력을 제공할 수 있는 중재 방법으 로 사용될 수 있을 것이다.

영문 초록

Objective: This study investigated the effects of wheelchair skills training on the wheelchair skill test (WST) score, wheelchair propulsion velocity (WPV), and activities of daily living in patients after an acute stroke. Methods: Eighteen inpatients were randomly allocated to either the experimental group, who underwent wheelchair skills training, or the control group, who underwent static standing training (n = 8 each). Each intervention comprised 30-min daily sessions, three times a week, for three weeks. To measure the wheelchair skills ability, WPV, and performance capacity in activities of daily living, the WST, WPV, and Korean modified Barthel Index (K-MBI) were implemented before and after the intervention. Results: After the intervention, both groups exhibited significant increases in the WST score, WPV, and K-MBI score (p < .05). Upon comparing the change in values, the experimental group showed a greater increase in the WST score and WPV (p < .05). Conclusion: Wheelchair technology training is effective in improving wheelchair skills and wheelchair propulsion speeds in patients who are unable to move independently after an acute stroke. Therefore, wheelchair skills training programs may be administered as an early intervention to provide safe and effective locomotion capabilities for patients after a stroke who cannot move independently.
