가정환경개선 평가도구에 대한 체계적 고찰

국문 초록

목적 : 본 연구는 지역사회 내에 거주하는 노인을 대상으로 하는 가정환경 평가도구를 체계적으로 고찰하 고, 각 평가도구의 특성 및 평가항목의 세부 내용을 종합적으로 분석하여 정리하고자 한다. 연구방법 : 2000년부터 1월부터 2019년 12월까지 Embase, NDSL, PubMed의 데이터베이스를 이용하여 가정환경 평가도구에 대한 논문을 검색하였으며, 1차 검색된 394편의 논문 중 선정기준 및 배제기준에 따 라 11편의 논문을 최종 선택하여 분석하였다. 결과 : 총 11편의 선정된 연구 중 근거수준 Ⅲ에 해당하는 단일 그룹 사전-사후 연구가 7편(63.6%)으로 가 장 많았다. 총 11편의 연구에서 사용된 가정환경 평가도구는 10개로 분석되었고, The Safety Assessment of Function and the Environment for Rehabilitation(SAFER-HOME)이 3편(23.1%)으로 가장 많았다. 가정환경 평가를 실시한 전문가는 작업치료사가 9편(81.8%)으로 가장 많았다. 본 연구에서 분석된 대부분 의 가정환경 평가도구는 가정의 실외 및 실내 낙상위험환경, 안전 및 재난 대피환경, 약물관리환경 등과 같은 환경적 요인을 평가하며, 일부 평가도구에서는 환경 내에서 수행하는 작업수행도가 포함되었다. 결론 : 총 11편에서 분석한 10개 가정환경개선 평가도구는 대부분 가정 내외의 낙상, 안전 및 재난 대피 환 경, 약물관리 환경 등을 평가하고 있었다. 특히 작업치료사에 의해 개발된 평가도구의 경우 환경요소 뿐 만 아니라 환경 내 작업수행도를 동시에 측정하는 것으로 나타났다. 각 평가도구의 평가항목은 서양식 주 거생활양식을 반영하고 있다는 점에서 한국형 주거생활양식을 반영하는 평가도구 개발이 필요할 것으로 사료된다.

영문 초록

Objective: This study aims to analyze the trends, item contents, and scoring system of a home safety assessment by systematically reviewing published articles on using home safety assessment tools. Methods: Articles on the home safety assessment of the elderly in the community published from January 2000 to December 2019 were searched using Embase, NDSL, and PubMed databases. Based on the selection criteria and exclusion criteria of the 394 articles, a total of 11 articles were finally selected and analyzed. Results: As a result, seven (63.6%) articles of the one group pre-and post-group studies of evidence level III were the most common. A total of 10 home environment assessment tools were used in 11 studies, and the Safety Assessment of Function and the Environment for Rehabilitation (SAFER-HOME) was the most commonly applied (23.1%). Occupational therapists (81.8%) were the most frequently asked to assess the home environment. Most of the home environment assessment tools analyzed in this study aimed to assess the risk factors of the home and the outside environment and to promote a safe occupational performance. Conclusion: A total of 10 home safety assessments were analyzed among 11 articles. They generally consist of items related to falls, safety, disaster plans, and medication environments. A home safety assessment by occupational therapists includes factors regarding occupational performance within the surrounding environment as well as the physical environment. The items in a home safety assessment reflected the house and lifestyle of western culture, and thus it will be necessary to develop our own culture-based home assessment in the future.
