뇌졸중 환자에서 전기자극 치료가 연하기능 회복에 미치는 연구

국문 초록

목적 : 뇌졸중 후 연하장애를 가진 환자에서 전기자극 치료가 연하기능 회복에 미치는 효과를 알아보고자 하였다. 연구방법 : 뇌졸중 후 연하장애를 가진 6명의 환자를 대상으로 15일 동안 1일 40분씩 두채널용 전기자극기를 이용하여 연하치료를 실시하였다. 1번 채널의 전극은 악이복근과 설골사이, 설골과 갑상 윤골 사이에 각각 부착시키고, 2번 채널은 갑상연골과 윤상연골 사이, 윤상연골 하방에 각각 부착 하였다. 반고형 식이를 이용하여 비디오투시검사를 치료전후에 두 번 시행하여 연하기능점수를 측정하였고 임상적으로 식이단계의 수준을 구하였다. 결과 : 임상적으로 식이단계의 수준 평가상 전기치료 후 타액조절단계에서 영양죽 섭취수준으로 기능의 향상을 보였으며(p<.05), 비디오 투시검사상 인두기 삼킴단계의 후두거상과 후두개 폐쇄, 후두계곡과 양배꼴동의 잔여정도, 후두통과시간에서 통계적으로 유의한 감소를 보였다(p<.05). 결론 : 전기자극 치료는 뇌졸중 후 연하장애를 갖는 환자에서 임상적으로 효과적인 치료방법으로 사료된다. 연하장애에 대한 전기자극 치료의 명확한 역할을 규명하기위해 향후 보다 많은 환자를 대상으로 후향성 대조군 연구가 필요하리라 생각된다.

영문 초록

Objective : To investigate the outcomes resulted from the use of electrical stimulation (ES) for swallowing function in stroke patients with dysphagia. Method : Six patients with dysphagia after stroke attack were examined. We used an electrical stimulator with two channels forty minutes a day for fifteen days. In the first channel, two electrodes were applied to the spots between digastricus and hyoid bone, between hyoid bone and thyroid cartilage, respectively. At the same time, in the second channel, two electrodes were applied to the spots between thyroid cartilage and cricoid cartilage, and below cricoid cartilage, respectively. We performed videofluoroscopic swallowing study (VFSS) twice, using semi-solid swallowing material for each patient, and assessed the degree of the response to ES through pre- and post-treatment functional dysphagia scales measured with the VFSS, which checked clinical swallowing stage. Result : After the ES treatment, there was significant improvement in clinical swallowing stage from saliva control to rice gruel control (p<.05). Also, significantly decreased scores were found on laryngeal elevation, epiglottic closure, residue in valleculae, residue in piriform recess and pharyngeal transit time for pharyngeal phase of swallowing (p<.05). Conclusion : The results showed that ES is a clinically effective intervention in treatment of the patients with dysphagia after stroke. Randomized controlled trials with more participants are needed to further clarify the role of ES on dysphagia.
