심한 개방교합과 짧은 치근을 가진 골격성 III급 부정교합의 수술 치험례

국문 초록

심한 개방교합을 가진 환자의 치료 시 개방 교합의 양상 및 환자 개개인의 상태에 따라 수술을 동반할지 또는 교정적으로만 치료할지를 결정해야 한다. 특히 전신 질환을 가진 환자는 치과적 문제뿐만 아니라 전신 질환이 치료에 미치는 영향 및 협조도에 대한 고려까지 포함한 치료 계획 수립이 필요하다. 심한 개방 교합으로 인해 장기간 기능을 하지 못한 치아는 치주인대의 위축성 변화가 일어나며 쥐를 이용한 동물실험에서도 치아 이동 시 hypofunctional periodontium group에서 더 큰 치근 흡수를 보인 바 있다.1 또한 교정 치료시 발생 가능한 치근 흡수는 기존의 치근 흡수가 있던 치아에서 더 큰 위험도를 가지며 치료 기간, 교정력의 크기와 적용 형태, 치아 이동 형태, 치아 이동량 등이 치근 흡수의 위험 인자로 알려져 있다. 본 증례에서는 전신 질환을 동반하여 매우 심한 개방교합과 매우 짧은 전치부 치근을 가지는 젊은 성인 환자를 소개하고자 한다. 상·하악 ASO를 동반한 악교정 수술을 통해 개방교합 및 골격성 III급 부정 교합을 해결하는 치료 계획을 세웠으나 수술 과정 중 불량한 상악 골질로 인해 예후가 의심되어 상악 ASO는 시행하지 못하였다. 이로 인해 술후 충분하지 않은 수직, 수평 피개가 형성되었으나 술후 교정 기간 동안 약한 힘으로 치아 이동을 유도하여 만족할 만한 수직, 수평 피개 형성 및 교합 형성을 하였다.

영문 초록

There should be a consideration for patients with severe openbite whether treatment should be accompanied by surgery or not according to the individual status of openbite severity and condition. In particular, patients with systemic diseases need treatment plans that include consideration of dental problems but also the effects of systemic diseases on treatment and degree of cooperation. Atrophic changes of periodontal ligaments occurred in the teeth that did not function for a long period due to severe open bite. In animal experiments using mice, larger root resorption occurred in the hypofunctional periodontium group after orthodontic tooth movement. In addition, root resorption that can occur during orthodontic treatment has a higher risk in the teeth with a pre-existed absorbed root. As it is known, the duration of treatment, the magnitude and application of orthodontic force, the type of tooth movement, and the amount of tooth movement are the risk factors related to orthodontic treatment for root resorption. This case shows a young adult skeletal Class III patient with very severe open bite and very short anterior root with systemic disease. Orthognathic surgery with bimax-illary anterior segmental osteotomy (ASO) was the original treatment plan for the patient, but the maxillary ASO was not performed due to the suspicion of prognosis of poor maxillary bone quality during the surgical procedure. This resulted in insufficient vertical and horizontal overbite after surgery and postoperative orthodontic treatment was performed with weak force to establish acceptable vertical and horizontal overbite, and occlusion.
