CRPS를 위한 작업치료 프로그램이 통증과 상지기능에 미치는 영향: 예비연구

국문 초록

목적 : 본 연구는 복합 통증 증후군(CRPS) 환자에게 작업치료 프로그램을 실시하였을 때, 중재 전. 후의 통증과 상지기능의 효과를 알아보고자 한다. 연구방법 : 연구는 2022년 1월부터 2022년 12월까지 서울특별시 A병원과 B병원에서 마취통증의학과의사에 의해 CRPS 진단을 받은 외래 환자 중 대상자 선정 기준에 적합한 환자 6명을 대상으로 1일 30분씩, 총 6주간 12세션으로 작업치료 프로그램을 진행하였다. CRPS 환자에게 작업치료 프로그램을 시행 전후에 Visual Analog Scale(VAS), Hand grip strength, Disabilities of the Arm, Shoulder, and Hand (DASH)를 사용하였다. 연구결과 : CRPS 중재프로그램 전 후 통증측정 VAS 점수는 7.1±1.8에서 4.7±2.3점으로 감소되었다. 기능측정에서 Grip strength는 9.4±7.9KG에서 16.3±10.7kg로 증가, Lateral pinch strength는 3.8±2.7kg에서 5.7±3.1kg로 증가, 3-point pinch strength는 2.9±1.6kg에서 4.7±2.5kg로 증가 하였고 DASH는 82.8± 8.1점에서 69.2±10.5점으로 감소하였다. 모든 기능에서 중재 후 통계적으로 유의한 차이를 보였다 (p<.05). 결론 : CRPS 진단을 받은 환자에게 맞춤형 작업치료 프로그램 시 통증감소와 상지기능 향상에 유의한 차이가 있었다. 본 연구를 통해 CRPS 환자에게 적극적인 재활에 참여하기를 기대한다.

영문 초록

Objective : This study aims to compare the effects of pain and upper limb function before and after intervention when an occupational treatment program was conducted on patients with complex regional pain syndrome (CRPS). Methods : From January 2022 to December 2022, six outpatients diagnosed with CRPS by an anesthesiologist at A and B hospitals in Seoul were treated in 12 sessions for a total of 6 weeks, 30 minutes a day. Visual Analog Scale (VAS), Hand grip strength, Disabilities of the Arm, Shoulder, and Hand (DASH) were used before and after the implementation of occupational therapy programs for patients with complex regional pain syndrome. Results : The pain measurement VAS score before and after the CRPS intervention program was reduced from 7.1±1.8 to 4.7±2.3 points. In the functional measurement, the Grip strength increased from 9.4±7.9KG to 16.3±10.7kg, the Lateral pinch strength increased from 3.8±2.7kg to 5.7±3.1kg, and the 3-point pinch strength increased from 2.9±1.6kg to 4.7±2.5kg. DASH decreased from 82.8±8.1 points to 69.2±10.5 points. There was a statistically significant difference after intervention in all functions (p<.05). Conclusion : There was a significant difference in pain reduction and improvement of upper limb function in customized occupational therapy programs for patients diagnosed with CRPS. Through this study, it is expected to actively participate in rehabilitation for CRPS patients.
