일본 장애인 돌봄서비스 가산제도와 시사점

국문 초록

본 연구는 우리나라의 중증장애인들이 돌봄 서비스 영역에서 소외당하는 문제를 해결하기 위하여, 일본의 중증장애인 돌봄 서비스인 중증방문개호제도 가산제도를 분석하여 시사점을 제시하는 데 목적이 있다. 연구방법은 문헌연구를 실시하였으며, 선행연구와 후생노동성 홈페이지를 기반으로 중증방문개호서비스가산, 특정사업소가산, 그리고 개호직원처우개선가산 제도의 내용을 분석하였다. 분석 결과 중증방문서비스가산에서는 서비스 강도와 중증장애인 배려 노력, 특정사업소가산에서는 장애인과 서비스 제공자에 대한 안정적인 지원체계 노력, 그리고 복지개호직원처우개선가산에서는 제공자가 전문 서비스를 제공할 수 있는 환경 조성 노력을 평가하고 있었다. 그 결과 중증장애인을 기피하기 현상보다는 이용자 확보를 위해 노력하는 사업소가 증가하고 있음을 알 수 있었다. 이러한 분석을 바탕으로 우리나라에서 중증장애인 다음과 같은 제언이 가능하다. 첫째, 장애 유형에 따라 서비스 영역이 구분되어야 한다. 둘째, 서비스 강도에 따라 서비스 단가에 차등을 두어야 한다. 셋째, 서비스 제공기관의 평가지표에 중증장애인 확보 노력을 평가 항목을 추가시키고. 현재의 격려금 차원의 금액을 급여를 올리는 수준으로 바꾸어야 할 것이다.

영문 초록

The purpose of this study is to analyze the extra-pay information in the home visiting nursing system for persons with severe disabilities in Japan and to make suggestions in order to address the alienation issue in the service to care for persons with severe disabilities in Korea. As a study method, literature review was conducted. Based on previous studies and the website of Ministry of Health, Labour and Welfare of Japan, the extra-pay of home visiting nursing service for persons with severe disabilities, the extra-pay of specific office, the introduction background of home visiting staff treatment improvement extra-pay system, the content of extra-pay, and the results of the system were analyzed. According to the analysis, in the extra-pay of home visiting nursing service, service intensity and the effort of consideration for persons with severe disabilities were evaluated; in the extra-pay of office, the effort of the stable support system for persons with disabilities and service providers was evaluated; in the extra-pay of staff treatment improvement, a service provider’s effort to establish the environment for providing professional service was evaluated. As a result, it was found that more offices strived to secure users, rather than to avoid persons with severe disabilities. Given the result, it is necessary to, first, categorize services areas according to disability types and offer different services prices depending on service intensity; second, to evaluate service offering institutions’ efforts to secure persons with severe disabilities and give them incentives; third, to differentiate education content in each nursing service area and raise home visiting nursing staff.
