ICF에 기반한 가정연계 재활 프로그램이 만성 뇌졸중 환자의 상지기능, 자기효능감 및 참여에 미치는 효과

국문 초록

연구목적 : 본 연구의 목적은 구조화된 가정연계 재활 프로그램이 만성기 뇌졸중 환자의 상지기능과 자기효능감 및 참여 수행능력에 미치는 효과를 알아보고자 하였다. 연구방법 : 만성기 뇌졸중 환자 20명을 대상으로 하였으며, 실험군 10명, 대조군 10명을 무작위로 나누어 연구를 진행하였다. 가정연계 재활 프로그램은 International Classification of Functioning, Disability and Health(ICF)의 신체기능 및 구조, 활동 및 참여, 법적 사회적 지원에 대한 정보 서비스와 관련된 활동을 바탕으로 하여 각 영역별로 구성하였으며, 작업치료를 받는 외래 환자를 대상으로 하루 30분, 주 5회(2회는 치료실, 3회는 가정), 8주 동안 실시하였다. 연구결과 : 연구결과, 중재 후 집단 내 상지기능 평가와 자기효능감척도 항목에서 실험군과 대조군 모두 통계적으로 유의한 향상을 보였다(p<.01). 참여의 수행도는 실험군에서만 개인관리, 이동성, 일상활동영역 총점, 사회역할영역 총점, 참여 전체 총점에서 유의한 향상을 나타내었다(p<.05, p<.01). 참여의 만족도는 실험군에서 개인관리, 일상활동영역 총점, 지역사회생활, 참여 전체 총점에서 유의한 향상을 보였고(p<.05, p<.01), 대조군에서는 일상활동영역 총점에서만 유의한 향상을 나타내었다(p<.05). 실험군과 대조군의 집단 간 변화량을 비교한 결과 상지기능, 자기효능감, 참여 수행도의 일상활동영역과 총점, 참여 만족도의 지역사회활동, 사회역할영역과 총점에서 유의한 차이가 있는 것으로 나타났다(p<.05, p<.01). 결론 : 구조화된 가정연계 재활 프로그램은 지역사회에서 거주하는 뇌졸중 환자에게 가정에서 할 수 있는 활동의 가이드라인을 제시했다는 것에 큰 의의가 있다. 또한, 추후 가정연계 재활 프로그램을 개발하고자 하는 치료사들에게 기초자료를 제공할 수 있을 것으로 기대한다.

영문 초록

Objective : The purpose of this study was to investigate the effect of a structured home-linked rehabilitation program on upper extremity function, self-efficacy, and participation performance in chronic stroke patients. Methods : The subjects were 20 chronic stroke patients, and 10 patients in the experimental group and 10 in the control group were randomly divided into the study. Based on the activities related to physical function, structure, activity, and participation in the International Classification of Functioning Disability and Health (ICF), the home-linked rehabilitation program was composed of 20 in each area, and 30 per day for outpatients receiving occupational therapy. Minute, 5 times a week (2 times in the treatment room, 3 times at home), 8 weeks. Results : As a result of the study, both the experimental group and the control group showed statistically significant improvement in the evaluation of upper extremity function and self-efficacy scale within the group after the intervention (p<.01). The performance of participation showed a significant improvement in personal management, mobility, daily activities, social role, and participation in the experimental group only (p<.05, p<.01). Satisfaction with participation showed a significant improvement in the total score of personal management, daily activity area, community life, and participation in the experimental group (p<.05, p<.01), and in the control group, only the total score of the daily activity area showed a significant improvement (p<.05, p<.01). shown (p<.05). As a result of comparing the amount of change between the groups of the experimental group and the control group, it was found that there were significant differences in the daily activity area and total score of upper extremity function, self-efficacy, and participation performance, community activity, and social role area and total score of participation satisfaction (p<.05, p<.01). Conclusion : The structured home-linked rehabilitation program implemented in this study has great significance in that it provided guidelines for activities that can be performed at home to stroke patients living in the community. In addition, it is expected to be able to provide basic data to therapists who want to develop a home-linked rehabilitation program in the future.
