소아암 생존자의 건강 관련 요구 측정도구 개발

국문 초록

소아암의 치료율이 높아지게 되면서 소아암 생존자 수가 증가함에 따라 소아암 생존자의 필수적인 건강 관련 요구도 증가하고 있다. 그러나 국내 소아암 생존자의 건강 관련 요구의 측정은 잘 이루어지지 않고 있다. 이에 본 연구는 우리나라 소아암 생존자의 건강 관련 요구를 측정할 수 있는 도구를 개발하기 위하여 진행되었다. 소아암 생존자를 대상으로 광범위한 문헌고찰과 면담을 통하여 도구를 개발하였고 소아암 생존자 371명을 대상으로 요구를 측정하여 도구의 타당도와 신뢰도를 검증하였다. 이러한 과정을 거쳐 개발된 최종 도구는 신체적 요구 영역 5문항, 정보 및 의료 시스템 요구 영역 11문항, 심리적 요구 영역 5문항, 사회적 요구 영역 4문항, 암 관련 심리사회적 지지 요구 영역 6문항, 재정적 지지 요구 영역 4문항, 긍정적 자기변화에 대한 요구 영역 6문항으로 최종 7개 영역의 41문항으로 구성되었다. 총 설명분산은 50.8%였으며, Cronbach’s Alpha는 .93으로 나타났다. 수렴타당도와 판별타당도는 각각 100%, 100%였다. 본 도구와 소아암 생존자의 삶의 질 측정도구의 피어슨 상관관계 계수가 .60으로 준거타당도가 수립되었다고 판단하였다. 본 연구를 통하여 개발된 도구는 국내 소아암 생존자의 건강 관련 요구를 측정하는 타당도와 신뢰도가 검증된 유일한 도구로 이를 사용하여 측정한 소아암 생존자의 다양한 요구가 반영된 간호 중재를 제공할 수 있을 것이며, 소아암 생존자의 다양한 연구를 수행하는데 기여할 것으로 기대한다.

영문 초록

Recently, the survival rates of childhood cancer have increased and the number of childhood cancer survivors is growing in South Korea. Childhood cancer survivors have much interests in their health-related needs, but health providers usually neglect these needs and there are no need scales for childhood cancer survivors in Korea. The purpose of this study was to identify health-related needs among childhood cancer survivors and to develop a scale to measure these needs. This study was proceeded by the two steps of development and testing. The final scale included 41 items comprising seven domains (Physical, informational & health care system, Psychological, Social, Cancer-related psychosocial support, Financial support, and Positive self-motivation need). A total of 371 pediatric cancer survivors were recruited from three children’s cancer center in Seoul, Korea. The total variance was 50.8%. Cronbach’s Alpha of items was .93. The convergent and determinant validity for this scale were 100.0%. The concurrent validity was assessed by examining relationships with quality of life. Significant correlations between the two scales were obtained. This scale is the first established in South Korea, which is valid and reliables. Therefore, the scale can provide scientific and empirical data to evaluate various kinds of needs and the level of needs for each survivor.
