The Effectiveness of an Art Therapy Approach for Diagnosis of Post-Traumatic Stress Disorder after Childhood Sexual Abuse

국문 초록

목적: 본 연구는 무서운 꿈을 꾼다는 이유로 의뢰된 8세의 여아가 미술치료적 접근을 활용한 사정기간을 통하여 급성 외상후 스트레스 장애(PTSD)로 진단받게 된 과정을 기술하였다. 방법: 실제 사례를 통한 질적 연구방식을 채택하였다. 초기면담 절차에서의 K-CBCL과 HTP 검사 결과 및 4회기의 사정 절차에서의 미술작업을 차례로 분석하여 DMS-IV-TR의 진단기준에 따라 진단하였으며, 각 회기의 그림을 기술하고 분석하여 제시하였다. 결과: K-CBCL과 HTP 검사 결과는 아동의 정서불안정을 증명하였으나 증상의 심각성만 제시할 뿐 그 원인이나 결정적인 단서를 제공하지는 못하였다. 그러나 미술치료적 접근을 통한 사정기간을 통하여 과거 성폭력 이력과 현재의 외상 재경험 요인이 드러나면서 미술치료 특유의 사정 양식은 보다 신속하게 PTSD 재진단과 체계적 개입에 조력하였다. 결론: 성폭력과 같이 외상성 사건이 밝혀지지 않고 많은 증상이 위장될 경우, PTSD 진단을 위한 구조적 면담에서의 미술치료적 접근은 증상과 그 원인의 틈을 메움으로써 아동 PTSD의 사정과 진단에 효과적인 도구로 작용하였다.

영문 초록

Purpose: This study described the process of diagnosing an 8-year-old girl suffering nightmares with acute post-traumatic stress disorder (PTSD) through a period of assessment using an art therapy approach. Methods: A qualitative research method was adopted for the case study. Analysis was performed on the Korean-Child Behavior Checklist, the House-Tree-Person Drawing test, and artwork created by the child during four assessment sessions, and a diagnosis was formed based on DSM-IV-TR criteria. The pictures drawn during each session were described and analyzed. Results: The results of the Korean-Child Behavior Checklist and a House-Tree-Person Drawing intake test indicated emotional instability but only showed the severity of symptoms, not their cause or any vital clues. Four assessment sessions of art therapy revealed that the child had a history of sexual abuse and was reexperiencing the trauma. A unique assessment modality, the art therapy approach allowed quicker diagnosis of PTSD and more effective systematic intervention. Conclusion: In cases when many symptoms of the abuse are camouflaged, an art therapy approach may be able to bridge the gap symptoms and cause when conducted during structural interviews intended to determine the presence of PTSD. An art therapy approach can be effectively used as a tool to assess and diagnose childhood PTSD.
