국문 초록
목 적 :
조현병은 초기 성인기에 발병하는 만성질환으로 큰 사회경제적 부담을 야기한다. 특히, 조현병은 만성적인 인지장애와 이로 인한 기능 장해가 지속되는 질환이며, 각종 대사성증후군 합병증에 취약하다. 현재까지 비정형 항정신병약물과 다양한 심리사회적 중재가 조현병 회복에 도움이 되는 것으로 알려져 있지만, 인지 손상과 기능 회복 영역에서는 그 효과가 제한적이다. 유산소 운동이 여러 생물학적 기전을 통해 뇌의 건강 수준을 향상시켜 조현병의 인지장애와 기능 회복에 도움이 될 것으로 추정되고 있다. 본 연구는 유산소 운동을 통한 인지장애 및 기능 손상 개선과 관련된 문헌을 살펴보고자 한다.
방 법 :
조현병 환자를 대상으로 한 유산소 운동 프로그램의 효과성을 검증하는 문헌을 구글학술검색과 Pubmed를 통해 검색하였다. 특히, 운동에 대한 인지장애에 대한 효과성을 검증한 연구를 우선으로 검색하였으며 되도록 MATRICS consensus cognitive battery(MCCB)를 활용한 문헌을 우선적으로 살펴보았다. 주간 운동 시간을 산출할 수 없는 문헌과 요가나 태극권만을 활용한 문헌은 배제하였다. 또한 임상증상, 인지기능, 수행기능과 관련된 평가도구를 활용하지 않고 주관적 만족도 등으로 운동 효과를 평가한 연구는 배제하였다.
결 과 :
유산소 운동을 통한 인지장애 개선 여부를 평가한 문헌은 총 11편이었으며, 그중 9편은 임상증상 변화 여부를, 6편은 기능 회복 여부에 대해 평가를 하였다. 인지기능 개선을 보고한 문헌은 총 8편으로 주로 작업기억과 사회적 인지기능과 언어적 학습 기능에 대한 개선이 확인되었다. 기능 회복 여부를 평가한 문헌 6편 중에서 유의미한 기능 개선을 확인한 문헌은 4편이었다. 또한 유의미한 임상증상의 개선을 검증한 문헌은 총 9편 중 7편으로 확인되었다. 일부 연구에서는 임상적 증상 개선과 생물학적 변화 간의 상관관 계도 입증하였다.
결 론 :
유산소 운동을 통해 조현병 환자의 임상증상과 인지장애를 개선하고 동시에 일상 생활의 기능 수행 수준을 개선할 수 있다. 다만, 치료적 효과를 담보하는 운동의 유형과 적정 강도와 시간과 기간에 대한 연구가 향후 더 필요하다.
영문 초록
Objectives :
Schizophrenia which commonly occurs at early adulthood causes great socioeconomic burdens. Schizophrenia is fea-tured by chronic cognitive impairment and persistent functional disability. And patients with schizophrenia is vulnerable to metabolic syndrome and its comorbidity. Atypical antipsychotics and various psychosocial interventions are helpful with the recovery, yet their efficacy is limited in aspects of cognitive impairment and functional disability. Regular aero-bic exercise is known to improve brain function with various mechanisms like neurogenesis, angiogenesis, and synapto-genesis. So it is plausible that aerobic exercise might exert its therapeutic effect on schizophrenia. We have searched the studies related to this issue.
Methods :
We conducted a search of Pubmed and Google Scholar to identify articles relating to therapeutic effects of aerobic ex-ercise in schizophrenia. We focused on the articles that examined the impact on cognitive impairment in schizophrenia using MATRICS Consensus Cognitive Battery (MCCB). And we also included the studies which utilized clinician-rat-ed instruments on cognitive, functional, or clinical domain. We excluded the articles that examined benefits of yoga or tai-chi only and failed to present the dose of exercise intervention.
Results :
We identified 11 articles that examined the impact of aerobic exercise on cognitive impairment in schizophrenia. 9 of them also examined change of clinical symptoms, and 7 of them showed significant improvement. 6 articles examined functional recovery and 4 of them showed significant improvement. 8 of 11 studies identified significant improvement of cognition, especially on working memory, social cognition, and verbal learning subsets. Some studies proved correlation between biological change and clinical improvement.
Conclusion :
Aerobic exercise can improve clinical and cognitive symptoms in schizophrenia, thus enhance level of functioning. However further investigation is warranted to determine the adequate dose and type of exercise for therapeutic purpose.
