국문 초록

최근 북한이탈주민이 급증하면서 북한이탈주민의 신체건강과 정신건강에 대한 관심도 커지고 있다. 북한이탈주민은 다양한 신체질환에 걸리기 쉽지만 탈북과정 중에는 제대로 된 치료를 받기 어렵다. 또한, 심리적 외상이나 남한 사회 적응 스트레스로 인해 외상후 스트레스 장애나 우울증과 같은 정신장애도 흔하다. 기존 연구에 따르면 북한이탈주민들의 신체건강은 정신건강에 강력한 영향을 주고 있으며, 이러한 신체건강과 정신건강의 밀접한 관련성은 남한 주민들에 비해 훨씬 두드러진다. 이는 1) 신체질환이 사회 적응에 미치는 악영향이 북한이탈주민에게 더욱 크기 때문일 수도 있으며, 2) 북한이탈주민들이 정신증상을 신체화나 신체증상 으로 표현하기 경우가 흔하기 때문일 수도 있으며, 3) 북한이탈주민들이 겪는 심리적 스트레스나 신경계질환이 동시에 신체질환과 정신장애를 일으키기 때문일 수도 있다. 북한이탈주민이 가지는 신체증상을 동반한 정신장애의 치료를 위해서는 정신과 외의 타 의료기관, 정신의료기관, 지역사회 정신보건기관의 연계가 필요하며, 이를 위한 장기적인 지원이 필요하다.

영문 초록

As North Korean defectors have rapidly been increased in South Korea, their physical and mental health has become an important issue. North Korean defectors are vulnerable to physical illnesses, and commonly have psychiatric illness such as posttraumatic stress disorder or depressive disorder. Interestingly, previous studies reported that the association between physical health and mental health in North Korean defectors were much more prominent than in South Koreans. This prominent association in North Korean defectors may be due to 1) stronger impact of physical illness on the adaptation to South Korea, 2) their frequent expression of somatization, and/or 3) psychological/physical stress as a common etiological factor for both physical and psychiatric illness. For the management of the psychiatric illness with somatic symptoms in North Korean defectors, long-term support for integrative approaches of psychiatric department, other medical department and community mental service is necessary.
