유리체강 내 트리암시놀론 주사 후 황반 허혈이 합병된 양측 광범위 매독 점상 내망막염

국문 초록

목적: 유리체강 내 트리암시놀론 주입 후 광범위한 점상 내망막염이 발병하여 안매독으로 진단 후 치료하였으나 황반허혈이 합병된1예를 다중 망막 영상 소견과 함께 보고하고자 한다. 증례요약: 69세 남자 환자가 첫 유리체강 내 트리암시놀론 주사 후 양안 시력이 안전수지로 저하되어 의뢰되었다. 안저검사에서 망막표층내 노란색 점상 침착물이 광범위하게 출현하였고 세동맥염과 유리체염이 동반되었다. 침착물 주위는 빛간섭단층혈관조영에서표층 및 심층 망막의 혈관밀도 감소와 형광안저혈관조영에서 국소 저형광으로 관찰되었다. 혈청매독검사에서 양성으로 판정되어 안매독에 동반한 점상 내망막염으로 진단하였다. 2주간 수용성 페니실린을 매일 2천 4백만 단위로 정주 치료 후 6개월 뒤 점상 내망막염 및 유리체염은 소실되었고 양안 교정시력은 20/100으로 호전되었다. 하지만 폐쇄성 혈관염으로 인한 모세혈관 비관류는 회복되지않아 황반허혈이 지속되었다. 결론: 광범위한 매독 점상 내망막염이 유리체강 내 스테로이드 주사 후 발생할 수 있고 폐쇄성 혈관염으로 인한 내망막허혈로 시력저하가 지속될 수 있다. 첫 안구 내 스테로이드 주사 치료가 계획된 환자는 매독 선별검사를 추천한다.

영문 초록

Purpose: To report a case of extensive syphilitic punctate inner retinitis (SPIR) triggering bilateral macular ischemia after intravitreal triamcinolone injections, and the multimodal retinal imaging findings. Case summary: A 69-year-old male patient with nonproliferative diabetic retinopathy was transferred to our hospital because of bilateral visual deterioration (to counting fingers) after the first intravitreal triamcinolone injection. Fundus examination revealed numerous yellow punctate precipitates in the superficial retinae, retinal arteriolitis, and vitritis. The punctate lesions and surrounding retinal regions showed decreased vascular density on optical coherence tomography angiography, and focal hypofluorescence on fluorescein angiography. The patient was positive for all of the Venereal Disease Research Laboratory test, the fluorescent treponemal antibody-absorption test, and Treponema pallidum hemagglutination; we diagnosed bilateral SPIR. After treatment with aqueous crystalline penicillin solutions (24 million units per day for 14 days), the punctate lesions reduced but arteriolitis progressed to obliterative vasculitis. After 6 months, the bilateral SPIR and vitritis resolved, and the bilateral visual acuity improved to 20/100. However, inner retinal and macular ischemia persisted because of capillary nonperfusion attributable to obliterative vasculitis. Conclusions: Extensive SPIR can develop after an initial intravitreal steroid injection; the inner retinal ischemia and visual loss may persist after treatment because obliterative vasculitis develops. Therefore, patients scheduled for intravitreal steroid injections should be screened for syphilis infection.
