서사 의학의 철학적 기초로서 의학적 해석학의 가능성과 한계

국문 초록

서사 의학은 서사학과 문학 비평을 기반으로 하여 탄생하였으며, 그 외에도 문화 연구, 정신분석학, 인류학, 여성학, 장애학, 의료사회학, 생명의료윤리, 현상학, 해석학, 환자 중심 의료 등의 다양한 이론적, 실천적 자원을 흡수·소화하면서 발전해왔다. 그중에서도 현상학 및 해석학은 서사 의학의 철학적 기초로 최근에 많은 관심을 받고 있다. 그런 점에서 본 논문은 서사 의학의 철학적 기초로서 해석학, 즉 의학적 해석학의 가능성과 한계를 살펴보았다. 철학적 해석학이 그랬듯이 초기의 의학적 해석학은 환자의 증상과 징후, 이야기, 의료 차트 등을 해석하는 텍스트 해석학에 초점이 맞추어져 있었으나 점차 의료와 환자-의사 관계의 본질을 해석학적으로 정초하려는 존재론으로 확장되어 갔다. 특히 스베니우스의 의학적 해석학은 대화를 중심으로 한 가다머의 해석학을 바탕으로 의학적 해석학을 한 차원 높게 발전시켰다고 평가받고 있다. 하지만 서사 의학의 관점에서 볼 때 스베니우스의 의학적 해석학은 가능성 못지않게 한계도 존재함을 알 수 있다. 인식론에서 존재론적 차원으로 의학적 해석학의 초점이 옮겨가면서 서사의 역할이 축소되는 것이 그것을 설명한다. 따라서 의학적 해석학이 서사 의학의 철학적 기초로서 적절한 기능을 발휘하기 위해서는 이야기를 통한 정체성 형성, 지배적인 목소리의 발견과 그에 대한 비판, 소외된 목소리의 회복과 같은 서사의 역할을 적극적으로 담아내는 자기의 해석학이면서 동시에 비판적 해석학이어야만 할 것이다.

영문 초록

Narrative medicine was born on the basis of narratology and literary criticism, and has developed by absorbing various theoretical and practical resources such as cultural research, psychoanalysis, anthropology, gender studies, disability studies, medical sociology, biomedical ethics, phenomenology, hermeneutics, and patient-centered m edicine. Am ong them, phenomenology and hermeneutics have recently received much attention in that they are the philosophical foundations of narrative medicine. In this regard, this paper examines the possibilities and limitations of hermeneutics, that is, medical hermeneutics, as a philosophical basis of narrative medicine. As in the case of philosophical hermeneutics, early medical hermeneutics focused on text hermeneutics that interpreted patient’s symptoms, signs, stories, and medical charts, but gradually expanded to ontological hermeneutics to understand the essence of medicine and patient-doctor relationships. In particular, Fredrik Svenaeus’ medical hermeneutics, based on Hans-Georg Gadamer’s hermeneutics, is considered to have renewed medical hermeneutics. But from the perspective of narrative medicine, Svenaeus’ medical hermeneutics reveals certain limitations as well, especially in that the role of narrative has been reduced as the focus of medical hermeneutics shift from epistemology to ontological dimension. Therefore, in order for medical hermeneutics to play an appropriate role as a philosophical basis for narrative medicine, it must be self-hermeneutics and critical hermeneutics that actively embraces narratological strategies such as the forming identity through stories, discovering and criticizing dominant voices, and recovering marginalized voices.
