의학의 지평을 확장시켜주는 위약효과

국문 초록

위약효과는 활성 성분이 없는 위약투여나 무해한 의료적 시술임에도 불구하고, 우리 몸에서 긍정적 효과의 치유가 일어나는 이상한 현상이다. 현대의학의 근간인 생의학은 위약효과의 생성 기전을 해명하지 못하고 있다. 생의학 기반의 현대의학이 질병 치료에 괄목할 만한 성과를 보여주었지만, 많은 문제점을 안고 있다. 이 글은 위약효과를 활용하여 현대의학의 문제점 개선에 기여하고자 먼저 위약효과의 위상을 현대의학의 두 축인 임상치료와 임상시험에서 간략히 살펴보았고, 이어서 생의학과 위약효과의 속성들을 각각 상세히 파악하였으며, 위약효과의 활용 가능성도 고찰하였다. 생의학과 상반적인 속성의 위약효과는 생의학 모델로는 설명이 불가능하다. 생의학은 생물학적인 몸을 대상화하여 질병의 특이적 표적을 겨냥하는 텍스트 중심적인 사유체계에 바탕을 둔 반면에, 위약효과에서는 생물학적 몸뿐만 아니라 ‘살아 있는 몸’을 중심으로 텍스트를 넘어 맥락의 영역까지도 다루고 있다. 기예-중심적, 질환-중심적, 경험적, 체화적, 수행적, 맥락적 측면들처럼 포괄적인 의학을 반영하는 위약효과의 특성들은 활용 잠재력이 지대하다. 위약효과의 연결적 속성들도 서로 상이한 영역들 간에 소통의 매개체 역할이 기대된다. 현대의학의 골칫거리로 치부되던 위약효과는 그간 우리에게 익숙해왔던 의학보다 더욱 지평이 확장된 새로운 의학의 세계로 여행을 허락해 주는 여권 같은 존재일 것이다.

영문 초록

Placebo effects are weird phenomena in which the healing of beneficial effect happens in our body in spite of the treatment of placebo, which has no active substances, or inert medical procedures. Biomedicine, basis of modern medicine, dose not explain how placebo effects are generated. Although biomedicine-based modern medicine has produced the outstanding achievements in the treatment of diseases, it has many problems. In order to contribute to the improvement of problems in modern medicine by utilizing placebo effects, the status of placebo effects was first examined in both fields of clinical practices and clinical trials, which are two main axis of modern medicine. Then the properties of biomedicine and placebo effects were in detail investigated respectively and the possible usefulness of placebo effects was also considered. Placebo effects, which are contrary to the characteristics of the biomedicine, can not be explained by biomedical models. While biomedicine is based upon the text-centered thinking system which aims the specific target of diseases by objectifying the biological body, many contextual domains beyond text around the lived body are in placebo effects treated in addition to the biological body. The properties of placebo effects such as art-centered, illness-centered, experiential, embodied, performative, and contextual aspects reflect the comprehensive medicine, indicating the vast utilizable potential. The connective characteristics of placebo effects could be also used as the medium of communication among different realms. Placebo effects dismissed as a nuisance in modern medicine might be the passport to the new world of medicine, which has more expanded perspectives than the medicine familiar with us to date.
