'개별 맞춤 의학'의 철학적 의미 고찰

국문 초록

현대의학의 최첨단을 이끌어 가고 있으며 미래 의학의 주축으로 기대되고 있는 "개별 맞춤 의학"은 개개 질병의 특성과 환자 개개인의 유전학적 또는 후성유전학적 특성을 반영하는 특정한 표적을 대상으로 개별적인 맞춤형의 치료를 도모하고 있다. 이는 과학 기반 현대의학의 강력한 성공사례이다. 그러나 현행 "개별 맞춤 의학"은 주로 객관적인 측면이라 할 수 있는 생물학적이고 신체적인 차원의 다양성을 개별 맞춤화하는 수준에 머무르고 있는 실정이다. 객관적인 지표 이외에도 환자의 질병상태와 치료과정에 영향을 미치고 있는 주관적 측면이나 여러 다양한 차원들의 개별성까지도 고려한 온전한 개별 맞춤화에는 매우 미흡하여 그 이름에 걸맞게 보다 완성도가 높은 "개별 맞춤 의학"에 이르기까지는 아직 과도기적인 단계에 있다. 이러한 문제점의 중요한 원인으로 저자는 현대의학에 있어서 "개별 맞춤 의학"을 뒷받침하고 있는 철학적 사상들과의 관련성을 제시하고 있다. 따라서 본 글에서는 현행 "개별 맞춤 의학"이 당면하고 있는 문제점들을 먼저 살펴본 다음에, 이러한 문제점에 대한 개선 방안을 모색하고 온전한 "개별 맞춤 의학"의 실현에 도움을 주고자 "개별 맞춤 의학"의 저변에 흐르는 철학적 개념들과 그 의미를 고찰하였다. 이를 바탕으로 온전한 "개별 맞춤 의학"의 실질적인 구현을 위하여 기본적으로 요구되는 사항들과 앞으로의 과제에 대해 체계적으로 제시하고자 하였다.

영문 초록

‘Personalized medicine’, which leads the state-of-the-art therapy of modern medicine and is expected to play a key role in future medicine, is trying individually tailored treatment for specific targets reflecting genetic, genomic, epigenetic, or epigenomic characteristics of each disease and each patient. It is one of successful examples of science-based modern medicine. However current ‘personalized medicine’ is in a state of personalizing variability in biological or physical dimension, which deals mostly with objective aspects. The present ‘personalized medicine’ remains insufficient to reach a more perfect personalization reflecting the subjective aspects and individualities of various dimensions, which exert influence on disease status and therapeutic process of each patient, in addition to objective indices. Therefore the current ‘personalized medicine' is still in the transition period to be more completed one commensurate with its name. The author suggest that the important reason for this problem is related with philosophical thoughts underpinning ‘personalized medicine’. This study therefore examine first the problems of current ‘personalized medicine’ and then contemplate the underlying philosophical concepts and their meaning of ‘personalized medicine’ in order to seek ways to improve the problems and aid the realization of complete ‘personalized medicine’. Based upon these results, basic requirements for the substantive implementation of complete ‘personalized medicine’ and future tasks are systematically presented.
