IOPI 기기를 이용한 구강안면근 훈련이 마비말장애가 있는 급성기 뇌졸중 환자의 조음체계에 미치는 효과: 예비연구

국문 초록

연구는 급성기 뇌졸중으로 인한 마비말장애가 있는 환자에게 IOPI 기기를 적용한 구강안면근 훈련이 마비말장애의 조음체계 변화에 미치는 영향을 알아보기 위한 예비연구로 시행하였다. 연구대상자는 연구 조건이 충족되는 급성기 뇌졸중 환자 14명으로 IOPI 기기를 이용한 구강안면근 훈련을 한 실험군 7명과 설압자를 이용한 구강안면근 훈련을 한 대조군 7명으로 진행하였다. 사전평가 후 3주 동안 구강안면근 훈련을 시행하고 사후평가를 통해 두 군을 비교하였다. 본 연구결과 IOPI 기기를 이용한 집단이 조음기관의 기능과 총점에서 유의한 향상을 보였다(p<.05). 또한 IOPI 기기를 이용한 집단이 /퍼/와 /커/의 반복운동속도, /퍼터커/의 일련운동속도에서 유의한 향상을 보였다(p<.05). 마지막으로 모음의 포먼트를 비교한 결과 IOPI 기기를 이용한 집단이 /아/의 F1, F2, /우/의 F2에서 유의한 변화를 보였다(p<.05). 이는, IOPI 기기를 이용한 구강안면근 훈련이 마비말장애 환자의 조음치료에 효과적인 중재방안이 될 수 있으며 이를 위한 후속연구가 필요하다는 것을 시사한다.

영문 초록

This study was conducted to pilot study to investigate the effect of oral facial muscle training using IOPI device on the change of articulation system of dysarthria in patients with dysarthria due to acute stroke. The subjects of the study were 14 acute stroke patients who met the study conditions and 7 subjects in the experimental group who trained the oral facial muscles using the IOPI device and 7 subjects in the control group who trained the oral facial muscles using the tongue depressor. Oral facial muscle training was performed for 3 weeks after pre evaluation and the 2 groups were compared through post evaluation. As a result of this study the group using the IOPI device showed significant improvement in the function and total score of the oro-motor mechanism (p<.05). In addition the group using the IOPI device showed a significant improvement in the alternation motion rate of /pʌ/, /kʌ/ and sequential motion rate of /pʌtʌkʌ/ (p<0.5). Finally as a result of comparing the formant of the vowels, the group using the IOPI device showed significant changes in F1, F2 of /a/, F2 of /u/ (p<.05). This suggests that oral facial muscle training using an IOPI device can be effective intervention method for articulation treatment of patients with dysarthria and follow-up studies are need.
