치매와 경도인지장애 노인의 다중언어 사용에 관한 체계적 고찰

국문 초록

본 연구는 다중언어 사용에 따른 치매와 경도인지장애의 발병 지연효과와 인지적 예방 효과에 대한 후향적 연구들을 체계적 고찰 하였다. 자료검색은 MEDLINE, CINAHL, Cochrane Library, PubMed의 데이터베이스를 사용하였다. 분석대상 연구는 총 10편이었으며, 방법론적 질 평가, PICOS 방법을 적용하여 체계적으로 정리하였다. 분석결과, 발병연령에 대한 지연효과는 증상 및 진단시점(최대 7.4년)과 연구보고시점(최대 7.1년) 모두 단일언어군보다 다중언어군의 연령이 더 높은 것으로 나타났다. 그러나 발병연령과 관련된 잠재적 요인들의 영향과 인지적 예방 효과에 대한 유의미한 결과는 일관되지 못한 것으로 나타났다. 이에 따라 모국어 이외에 다른 언어를 구사하는 것은 치매와 경도인지장애 발병을 지연시킬 수 있는 것으로 나타났으며, 긍정적인 결과가 우세한 것으로 볼 수 있다. 그러나 이와 관련된 잠재적 요인과 인지적 예방에 대한 이점은 명확한 결론을 짓기 어려운 실정이었다. 향후 다중언어 사용에 따른 발병연령에 영향을 주는 잠재적 요인을 구분하고, 언어구사 능력에 따른 효과성을 분석할 필요가 있다.

영문 초록

This study systematic reviewed retrospective studies on the age of onset and cognitive reserve of the elderly with dementia and Mild Cognitive Impairment(MCI) due to multilingual use. For data search, databases of MEDLINE, CINAHL, Cochrane Library, and Library, and PubMed were used. A total of 10 studies were included in the analysis, and the data were extracted and organized systematic by quality assessment and PICOS. As a result, the delay effect on the age of onset was found to be higher in the multilingual group than in the monolingual group at both the time of symptom and diagnosis(up to 7.4 years) and the time of research report(up to 7.1 years). The effects of potential factors related to age of onset and the significant results of effectiveness on cognitive were found to be inconsistent. Accordingly, it has been shown that speaking a language other than the mother tongue can delay the onset of MCI and dementia, and it can be seen that the positive result prevails. However, it was difficult to draw a clear conclusion about the potential factors and the benefits for cognitive. In the future, it is necessary to classify potential factors affecting the age of onset according to the use of multilingual and analyze the effect according to language proficiency.
