뇌성마비 아동의 일상생활 참여에 영향을 미치는 요인

국문 초록

이 연구는 뇌성마비 아동의 일상생활 참여에 영향을 미치는 요인을 알아보기 위해 구조방정식 모형 검증을 실시하는데 그 목적이 있었다. 이를 위해, 만6~18세 뇌성마비 아동 176명을 대상으로 신체 구조와 기능, 맥락, 활동 및 참여 빈도를 측정하였다. 대동작 기능 분류 체계, 소동작 기능 분류 체계, 아동용 일상생활활동 측정 도구, 국립특수교육원 적응행동 검사, 한국판 부모양육 스트레스 검사, 참여 빈도 질문지가 도구로 사용되었다. 신체 구조와 기능은 수반장애, 인지, 적응행동으로 구성되었으며, 활동은 대동작 기능과 소동작 기능으로 구성되었으며, 맥락은 사회경제적 상태와 부모양육 스트레스로 구성되었다. 측정 모형의 적합도 검증 결과 측정 모형은 적합한 것으로 나타났으며, 구조방정식 모형의 적합도 역시 적합한 것으로 나타났다. 활동과 맥락은 일상생활 참여에 직접적 영향을 미치는 것으로 나타났고 신체 구조와 기능의 수준은 일상생활 참여에 활동과 맥락을 통해 간접적 영향을 미치는 것으로 나타났다. 활동의 수준이 낮을수록 참여 수준도 낮아지는 것으로 나타났으며(β = -.284), 사회경제적 상태 및 부모양육스트레스 등이 낮을수록 참여 수준은 높아지는 것으로 나타났다(β = -.559). 신체 구조와 기능, 활동 및 맥락이 참여를 설명하는 설명량은 51.0%인 것으로 나타났다. 이 연구는 뇌성마비 아동의 일생생활 참여에 영향을 미치는 요인에 대한 모형을 구조방정식을 통해 검증하여 요인 간의 관계에 대한 결과를 제시하였다는데 그 의의가 있다.

영문 초록

The purpose of this study was investigate the factors contributing on participation in everyday activities for children with cerebral palsy(CP). This study confirmed the measurement model consisted of body structure and function, activities and context and performed structural equation modeling(SEM) between body structure and function, activities, context and participation in everyday activities. 176 children (106 boys, 70 girls) with CP participated in this cross-sectional study. The WeeFIM and Korea Institute for Special Education-Scales of Adaptive Behavior used to assess body structure and function. The Gross Motor Function Classification System and Manual Motor Function Classification System used to assess activities and Korean Parent Stress Index used to assess context and Frequency of Participation Questionnaire used for participation in everyday activities. The construct of body structure and function, activities, and context was confirmed though an examination of a measurement model. The proposed SEM model showed good fit indices. Activities and context factor have direct effect on participation in everyday activities and body structure and function have indirect effect on participation though activities and context. Activities context effected participation (β = -.284 and -.559). Participation in everyday activities was explained 51.0% by body structure and function, activities, and context. The implication of study was that the structural equation modeling of factors contributing on participation in everyday activities was proposed in children with CP.
