노인의 피학대 경험과 자기방임의 관계

국문 초록

본 연구는 타인에 의한 노인의 피학대 경험이 우울을 매개하여 자기방임에 미치는 영향에 있어 사회적 지지의 조절효과를 검증하는 것을 목적으로 한다. 연구목적의 달성을 위해 2017년 노인실태조사 자료를 활용하였고, 선행연구 고찰을 통해 매칭변수를 선정, 성향점수를 추정한 뒤 매칭하여 피학대 경험이 있는 노인 989명과 유사한 특성을 가진 통제집단 816명에 대해 분석을 실시하였다. 분석 결과, 피학대 경험이 있는 노인은 우울수준이 더 높으며 더 많은 자기방임 행위의 위험이 있는 것으로 나타났다. 또한 피학대 경험이 우울을 매개하여 자기방임을 증가시키는 경로가 유의하였으며, 우울의 부분매개효과가 검증되었다. 마지막으로 사회적 지지는 우울이 자기방임에 미치는 영향을 감소시켜주는 조절효과를 가지고 있었다. 연구결과를 바탕으로 피학대 노인에 대한 주기적인 심리사회적 건강 사정, 학대 피해 노인의 사회적 지지체계 구축을 위한 개입 프로그램 마련, 학대피해노인전용쉼터 등에서의 자기방임 방지 및 자기돌봄 프로그램 개발을 제언하였다.

영문 초록

This study aims to investigate relations between elder maltreatment by others and self-neglect, with depression as a mediator moderated by social support. Using the 2017 National Survey of Elderly Koreans data, 989 people who have been maltreated or neglected by others and 816 who have not were extracted and matched by estimated propensity scores. Multiple regression analysis results show how maltreated elders tend to have more severe level of depression than the control group, while depression mediates maltreatment and self-neglect. It was also examined that and depression have significant effects on self-neglect tendency, while social support works as a moderator, mitigating effects of depression on self-neglect. The both indirect effect from maltreatment through moderated depression to self-neglect and total effect of maltreatment on self-neglect were statistically significant. Based on the results, regular intakes for elders, interventions to rebuild social support system for maltreated elders, and programs to be developed for elders empowerment and preventing self-neglect were suggested.
