당뇨망막병증 환자에서 후낭혼탁 제거 위한 Nd:YAG레이저 술 후 발생한 망막앞출혈 1예

영문 초록

Purpose: We report one case of preretinal hemorrhage following neodymium-doped yttrium aluminium garnet (Nd:YAG) laser posterior capsulotomy for posterior capsule opacification in diabetic retinopathy patient. Case summary: A 54-year-old male diabetic retinopathy patient was refered to our clinic complaining of abruptly decreased visual acuity (OD) following Nd:YAG laser posterior capsulotomy for posterior capsule opacification in pseudophakic right eye. At that time best corrected visual acuity of the right eye was finger count at 10 cm. On fundus examination there was preretinal hemorrhage in posterior pole including macula. We immediately treated with intravitreal injections of tissue Plasminogen Activator (tPA, 25 IU) and Sulphurhexafluoride (SF6) gas (100%, 0.6 cc). After 9 days, fundus examination shows no improvement of vitreous hemorrhage. So we performed a pars plana vitrectomy and endophotocoagulation. During the operation, we observed a retinal injured area, in inferotemporal retina, which was presumed that injury was occurred by Nd:YAG laser. One month after the surgery, preretinal fibrosis like proliferation formation was seen on fundus examination and postoperative 5 months best corrected visual acuity was 0.16. Conclusions: Nd:YAG laser posterior capsulotomy is effective method in treatment for posterior capsule opacification, but this method can lead to accidental retinal injury resulting in decreased visual acuity, especially in diabetic retinopathy patient.
