엔테로박터 클로아카에 의한 안내염 1예

국문 초록

목적: Enterobacter cloaca는 위장관에 정상세균총으로 상재하는 그람음성간균으로 면역저하된 조건에서 기회감염을 유발하는 것으로 알려져 있는데, 세계적으로 총 8예만 보고된 바 있는 Enterobacter cloacae 안내염 1예를 국내 최초로 경험하였기에 저자들은 문헌고찰과 함께 보고하는 바이다. 대상과 방법: 47세 남자환자가 예초기로부터 돌이 튀어 좌안의 공막열상으로 내원 하여 당일 응급 공막 일차 봉합술을 시행받았다. 술 전 시력은 0.2이었으나 술 후 1일 시력이 안전수지로 저하되면서 세극등 검사에서 좌안의 결막충혈과 각막 부종 및 전방내 염증세포와 전방축농을 보였으며 술 후 2일부터는 안초음파에서 저반사되는 유리체강내 막들이 관찰되어 안내염 진단하에 유리체강 내 항생제주사 및 유리체절제술을 시행하였다. 함께 시행한 균동정에서는 Enterobactor cloaca로 확인되었다. 술 후 2개월 뒤 최종 시력은 안전수지로 저하되어 있었다. 결과: 적극적이고 신속한 치료에도 불구하고 다른 그람음성간균들에 비해서 시력예후가 좋지 않은 특징을 가진 Enterobacter clocae로 인한 안내염에 대한 주의가 필요하다.

영문 초록

Purpose: Enterobacter cloacae are commensal gram-negative organisms of the gastrointestinal tract and are considered pathogenic only for patients with lowered resistance to infection. We experienced a case of endophthalmitis caused by the gram-negative bacillus, Enterobacter cloacae which has not yet been reported in Korea. Methods: A 47-year-old male had been hurted his left eye by a foreign body flew from mower revealed 3 mm sized scleral laceration near the limbus and best-corrected visual acuity (BCVA) of 20/100. On the day, He underwent primary closure of scleral laceration. A day after primary closure of surgery, the visual acuity had dropped to hand movement. Ocular findings such as corneal edema, hypopyon with fibrinous pupillary membrane in the anterior chamber and multiple low reflective vitreous membrane shown by ultrasonographic scan were consistent with infectious endophthalmitis. We provided intravitreal vancomycin and ceftazidime injection and vitrectomy. Culture of vitreous aspirate isolated Enterobacter clocae 6 days after traumatic event. Despite every effort, visual outcome in his left eye was handmotion 2 months postoperatively. Results: Clinicians should be aware of the devastating effects of endophthalmitis which is caused by Enterobacter clocae.
