국문 초록

안전한 진료환경 조성은 의료인을 비롯하여 국민 건강과 직결되는 문제로 사회적 위험관리 차원에서 접근이 요구된다. 정부는 환자의 안전, 건강보호 및 응급실 폭행 예방을 위해 환자 안전관리 체계를 확립하고 응급실 안전관리 강화를 추진하고 있다. 그러나 환자 안전 강화 및 응급실 폭행 예방을 위한 대책을 수립·시행하고 있음에도 불구하고 의료인 폭행사고가 꾸준히 발생하고 있다. 따라서 안전한 진료 환경 조성은 미흡한 상황이라고 볼 수 있다. 2018년 강북삼성병원의 환자 진료 중 발생한 사망사고의 발생으로 의료기관 전반의 안전시스템 개선 필요성이 더욱 제기되고 있다. 이 연구는 위험의 기본요소를 토대로 의료기관의 안전 강화를 위한 제언을 시도하는데 목적이 있다. 연구는 문헌 조사 및 공식적 자료의 분석을 토대로 수행되었다. 또한 의료기관의 안전을 위협하는 요인과 특성을 검토하고, 위험 현황과 사례를 분석하였다. Abkowitz가 제시하고 있는 위험의 기본요소를 고려한 제언은 다음과 같다. 첫째, 법·제도적 결함의 최소화가 필요하다. 둘째, 적극적 유지보수 활동을 위한 상시 점검 시스템 마련이다. 셋째, 유연화된 경제적 지원이 확대되어야 한다. 넷째, 수요조사를 기반한 교육훈련의 다양화가 필요하다. 다섯째, 위기 상황에 대한 시나리오별 매뉴얼 개선이 필요하다. 여섯째, 위험 상황에 대한 원활한 의사소통 채널이 강구되어야 한다. 일곱째, 현장경찰활동이 강화되어야 한다. 끝으로, 보건의료기관 종사자 및 안전요원, 환경미화 담당자에 대한 보호제도가 마련되어야 한다.

영문 초록

The creation of a safe medical care environment is directly related to public health, including medical personnel, and requires access to social risk management. The government has established a patient safety management system to promote patient safety, health protection and emergency room assault, and is promoting the strengthening of emergency room safety management. However, despite the establishment and implementation of measures to strengthen patient safety and prevent assault on the emergency room, assault incidents are still occurring. Therefore, it can be considered that the creation of a safe medical environment is insufficient. The need to improve the safety system of all medical institutions has been raised due to the death of patients in Gangbuk Samsung Hospital during 2018. The purpose of this study is to attempt to make suggestions for enhancing safety in medical institutions based on the basic factors of risk. The study was conducted based on literature surveys and analysis of official data. In addition, factors and characteristics that threaten the safety of medical institutions were reviewed, and risk conditions and cases were analyzed. The following are recommendations that take into account the basic elements of risk presented by Abkowitz. First, it is necessary to minimize legal and institutional defects. Second, a regular inspection system should be established for active maintenance activities. Third, flexible economic support should be expanded. Fourth, it is necessary to diversify education and training based on demand surveys. Fifth, it is necessary to improve the manual for each scenario considering the crisis situation. Sixth, a smooth communication channel for dangerous situations should be established. Seventh, On-site police activities should be strengthened. Finally, protection systems should be in place for health workers and safety personnel and sanitation workers.
