노인요양시설 문화변화 모델 적용의 효과성 연구

국문 초록

본 연구의 목적은 노인요양시설의 문화변화 모델 적용이 시설거주노인에게 미치는 긍정적 영향을 실증적으로 검증하는 것이다. 1차년도는 4개 요양시설별로 요양인력을 대상으로 문화변화역량 강화 프로그램을 제공하고, 시설의 문화변화를 개선할 수 있는 실행 방안을 도출하였다. 2차년도는 문화변화 실행 방안을 시설별로 실제로 적용하여 거주노인이 주관적으로 인식하는 삶의 질, 인간중심환경, 우울감, 고독감 등을 사전조사, 중간조사, 사후조사 하였다. 총 149명의 시설거주노인을 대상으로 조사했으며, 일원반복측정 분산분석을 활용하였다. 분석결과 삶의 질, 인간중심환경, 우울감 등이 통계적으로 유의하게 변화하였다. 삶의 질은 사전조사에 비교해서 중간조사와 사후조사가 통계적으로 유의하게 높았다. 인간중심환경은 사전조사와 중간조사에 비교해서 사후조사에서 통계적으로 유의하게 높았다. 우울감은 사후조사가 사전 및 중간조사에 비해 통계적으로 유의하게 낮았다. 반면 시설거주노인이 인식하는 고독감에 있어서는 유의한 변화가 발견되지 않았다. 이러한 연구결과를 토대로 시설거주노인의 삶의 질을 개선할 수 있는 노인복지실천 및 정책적 제언을 논의하였다.

영문 초록

The purpose of this study was to examine whether the adapted culture change model would have positive effects on lives of older residents or not. At the first year, the PI provided “Culture change capacity enhancement program”to four nursing homes, and the staffs in the facilities developed culture change strategies to improve person-centered care and home like environment. At the second year, the four nursing homes implemented the culture change strategies for the residents. While conducting the culture change practices about 12 months, we carried out the follow-up interviews (mid and post) with the same measures as we employed them before the intervention (quality of life, person-centered climate, depressive symptoms, and isolation). The One-Way Repeated Measures Analyses of Variance were performed with SPSS 26.0. A total of 149 older residents were participated. The results showed that quality of life, person-centered climate, and depressive symptoms were statistically significantly changed. The culture change intervention significantly improved the quality of life and person-centered climate of the residents. The depressive symptoms of the residents at post-test were significantly lower than that of at pre-test and at mid-test. However, the change in the isolation levels of the older residents were not statistically significant. Based on the findings of the study, gerontological practices and policies were discussed to improve quality of life of the older adults in nursing homes.
